Clinical Observation On Treatment Of Chronic Tension-type Headach With Shufengzhentong Decoction Based On The Theory Of Wind Medicine

Objective: Based on the theory of wind medicine,the clinical efficacy of Shufengzhentong decoction in the treatment of chronic tension-type headache with wind-fire disturbance was observed,which provided new ideas and reference for the clinical treatment of this disease.Methods:70 patients with chronic tension-type headache of wind-fire disturbance were randomly divided into two groups according to the inclusion criteria,with 35 cases in each group.The treatment group was treated with Shufengzhentong decoction plus or minus combined with eperisone hydrochloride.The control group was treated with eperisone hydrochloride.Before and after treatment,the scores of headache index,TCM syndrome score scale,Hamilton Anxiety Scale(HAMA)and Hamilton Depression Scale(HAMD)of the two groups were collected to observe their clinical efficacy.Results:The headache symptoms,some TCM syndromes and emotional state of the subjects in both groups were significantly improved compared with before treatment(P < 0.05),and the attack frequency,duration,degree of pain,accompanying symptoms,total score of TCM syndromes,part of TCM syndromes(headache,irritability,bitter taste in mouth,poor sleep and multiple dreams)and HAMD score were significantly improved than the control group(P<0.05).1.Headache index score:(1)Intra-group comparison:The total score of headache index and the score of each factor in both groups were significantly lower than those before treatment(P<0.05);(2)Comparison between groups:The total score of headache index and the score of each factor in the treatment group were significantly lower than those in the control group(P<0.05);(3)Headache effect:The total effective rate of headache index in the treatment group(93.9%)was significantly higher than that in the control group(78.1%),with statistical difference(P<0.05).2.TCM syndrome score:(1)Intra-group comparison:In the treatment group,the total score of TCM syndrome,headache,irritability,bitter taste in mouth,head(eye)swelling,hypochondriac pain,dry mouth,poor sleep and multiple dreams were significantly lower than those before treatment(P<0.05),There was no significant decrease in single score of flushing(P>0.05).In the control group,the total score of TCM syndromes and the single score of headache were significantly lower than those before treatment(P<0.05),There was no significant decrease in single score of irritability,bitter taste in mouth,head(eye)swelling,flushing,hypochondriac pain,dry mouth,poor sleep and multiple dreams(P>0.05);(2)Comparison between groups:In the treatment group,the total score of TCM syndromes and the single score of headache,irritability,bitter taste in mouth,poor sleep and multiple dreams were significantly lower than those in the control group(P<0.05),There was no significant difference between the groups in the single score of head(eye)swelling,flushing,hypochondriac pain and dry mouth(P>0.05);(3)TCM syndrome effect:The total effective rate of TCM syndrome in the treatment group(87.9%)was significantly higher than that in the control group(56.3%),with statistical difference(P<0.05).3.Emotional state score:(1)Intra-group comparison:The HAMA score and HAMD score of patients in the two groups after treatment were significantly lower than those before treatment(P<0.05);(2)Comparison between groups:The HAMD score of the treatment group was significantly lower than that of the control group(P<0.05),There was no significant difference in HAMA score between groups(P>0.05);(3)Effect of mood state:The effective rate of HAMD score in the treatment group(93.9%)was significantly higher than that in the control group(78.1%),with statistical difference(P<0.05);The effective rate of HAMA score in the treatment group(87.9%)was not significantly different from that in the control group(71.9%),with no statistical difference(P>0.05).Conclusion:The main pathogenesis of chronic tension-type headache is the loss of the liver,the transformation of the wind and fire,the fanning of the wind and fire,and the blocking of the brain orifices.In the treatment,the internal wind and internal fire are usually treated with wind medicine.In clinical practice,the Shufengzhentong decoction plus or minus has a significant effect on the treatment of this type of headache,which can effectively improve the headache symptoms,some TCM syndromes and depression of patients,and has no obvious adverse reactions during the treatment,which is worthy of clinical reference and promotion.
