| ObjectiveAutomatic strain echocardiography(AS)was used to quantitatively analyze the function changes of early left ventricular in overweight and obese patients,and to evaluate the value of AS in evaluating the early left atrial and left ventricular myocardial remodeling in these patients.MethodsA total of 144 healthy patients who underwent physical examination in our hospital from April 2021 to May 2022 were randomly selected and divided into three groups according to BMI: 24≤BMI<28 for overweight group,BMI≥28 for obesity group,18.5≤BMI<24 for control group(including 50 cases of overweight group,46 cases of obesity group,and 48 cases of control group).The general condition,blood biochemical examination and other disease history of the subjects were obtained through inquiry,measurement and hospital HIS system,and recorded in detail.The height and weight of the subjects were strictly measured by the computerized human scale in the physical examination center,and their BMI values were accurately obtained.The LA-ap and LAV,LAVI,LVEDV,LVESV,LVEF,and MV-E,MV-A,e’-MVsep,e’-MVlat were measured by two-dimensional transthoracic echocardiography in all three groups.The LASr,LAScd,LASct and LV GLS were measured by AS technique.All data collected will be recorded in detail and analyzed by statistical methods.Results1.There were no significant differences in gender,age,heart rate and blood pressure among the three groups(P>0.05).The BMI,waist circumference of overweight group were increased compared with the control group,and the differences were statistically significant(P<0.05).Body mass index,waist circumference were increased between obesity group and overweight group,and the differences were of statistical significance(P<0.05).Body mass index,waist circumference of obesity group and control group were considerably increased,and the differences has statistically significant(P<0.05).2.There were no significant differences in hemoglobin concentration,white blood cell count,red blood cell count,platelet count,fasting blood glucose and creatinine among the three groups(P>0.05).The difference of triglyceride among the three groups was statistically significant(P<0.05).There were statistically significant differences in total cholesterol,low density lipoprotein cholesterol,high density lipoprotein cholesterol and uric acid between control group and overweight group(P<0.05),statistically significant differences between control group and obesity group(P<0.05),but no statistically significant differences between obesity group and hyperrecombination(P>0.05).3.There were no significant differences in LA-ap,LAV max,LVEDV,LVESV and LVEF among the three groups(P>0.05).The difference of E/A among the three groups was statistically significant(P<0.05).There were statistically significant differences in LAVI,E/e’ between control group and overweight group(P<0.05),statistically significant differences between control group and obesity group(P<0.05),but not statistically significant between obesity group and overweight group(P>0.05).4.There were statistically significant differences in LAScd and LV GLS among the three groups(P<0.05).There was statistically significant in LASr between the obesity group and the control group(P<0.05),there was statistically significant between the obesity group and the overweight group(P<0.05),and there was no statistically significant between the overweight group and the control group(P>0.05).There was statistically significant in LASct between the obesity group and the control group(P<0.05),but no statistically significance between the overweight group and the obesity group(P>0.05),and no statistically significance between the overweight group and the control group(P>0.05).5.The BMI,WC,TG,TC,LDL,UA and LASr were all negatively correlated,while LASr was not correlated with HDL.The BMI,WC,TG,TC,LDL,UA and LAScd were all negatively correlated.There was a positive correlation between HDL and LAScd.The BMI,WC,TG,TC,LDL,UA,HDL and LASct were not correlated.The BMI,WC,TG,TC,LDL,UA and LV GLS were all negatively correlated.The HDL and LV GLS was positive correlation.6.Multiple stepwise regression analysis showed that BMI and TG were the main indicators affecting the LAsr,LAscd and LV GLS.BMI and TG are the main factors affecting the left atrial and left ventricular strain values.All variance inflation factors were less than 10.0,indicating that there was no collinearity among the influencing factors.7.The intra-group correlation coefficient(ICC)was used to analyze the intra-observer and inter-observer repeatability.The results showed that the ICC of LASr,LAScd,LASct and LV GLS were all greater than 0.75,and the reliability was all credible(P<0.05).Conclusions1.Automatic strain technique can quantitatively evaluate the early changes of left heart function in overweight and obese patients,and detect abnormal active cardiac movement earlier.2.The BMI and TG are independent influencing factors of left cardiac strain parameters. |
