PruposeAt present,the incidence of male infertility in our country has amounted to 10%-12%.Year by year,the rate of male infertility has grown,becoming a matter of public concern and garnering more and more attention.The factors that cause male infertility are congenital heredity and acquired,and the factors that cause male spermatogenic dysfunction can be divided into pre-testicular factors,testicular factors and posttesticular factors.When problems and changes occur in any link due to various pathogenesis and factors,the process of spermatogenesis is affected,and ultimately the quality of male semen and spermatogenesis function decline,resulting in the occurrence of infertility,so the quality of male semen itself is also the embodiment of spermatogenesis function.Therefore,we believe that the decrease of male spermatogenic function is the ultimate path of male infertility,and male fertility can be judged by testing the quality of male semen or indicators reflecting spermatogenic function.Previous studies have shown that serum IGF-1 may have a potential effect on spermatogenesis and fertility in men.Therefore,in this study,we evaluated serum IGF-1 levels in men with impaired spermatogenesis,including asthenospermia,oligospermia,teratospermia,oligoasthenospermia,and non-obstructive azoospermia,and in men with normal spermatogenesis.Bioinformatics related methods were used for statistical analysis.To explore whether serum IGF-1 level is correlated with male spermatogenic function and evaluate whether it can be used as a predictor of male spermatogenic function,in order to better evaluate male spermatogenic function and provide a new possibility for the clinical diagnosis and treatment of male infertility.Data and methodA total of 781 healthy men and infertile patients from the male outpatient department and laboratory of Reproductive Medicine Center of Southern Medical University Nanfang Hospital from January 2021 to December 2022 were retrospectively analyzed,including 62 healthy male samples with normal reproductive function,34 with oligospermia,32 with asthenospermia,and 32 with teratospermia.There were 153 patients with oligasthenospermia and 468 patients with non-obstructive azoospermia.Data such as age,weight,height and BMI were collected.Semen and serum samples were collected for routine analysis of semen and determination of serum IGF-1 concentration.Statistical processing:R Language(version 4.2.1)was used to complete the process.Analysis of the data was conducted using t test,variance analysis,and Pearson correlation analysis,with measurement data expressed as mean value and standard deviation.Statistical significance was determined to be P<0.05.ResultsThe mean serum IGF-1 concentration of normal reproductive function males was 123.0 ng/ml,the mean serum IGF-1 concentration of oligospermia patients was 11.1.2 ng/ml,and the mean serum IGF-1 concentration of asthenospermia patients was 99.0 ng/ml.The mean serum IGF-1 concentration was 108.4 ng/ml in patients with teratoospermia,101.8 ng/ml in patients with oligospermia,and 109.5 ng/ml in patients with non-obstructive azoospermia.P<0.05,the difference was statistically significant.A positive correlation(rpearson=0.33)was observed between serum IGF-1 concentration and forward motility of sperm in males with normal fertility function,with a linear relationship.P<0.05,the difference was statistically significant.ConclusionThere was a significant difference in serum IGF-1 concentration between men with normal fertility function and patients with abnormal semen parameters.The serum IGF-1 concentration was higher in men with normal fertility function,but decreased in patients with abnormal spermatogenesis function and may be lower than that in men with normal fertility function,which was 123.0 ng/ml on average.Serum IGF-1 concentration was positively correlated with sperm forward motility and sperm motility.The higher the forward motility of sperm,the higher the serum IGF-1 concentration.This study suggests that serum IGF-1 level can be used in clinical diagnosis of semen quality and spermatogenic function,especially in patients with asthenospermia.
