| BACKGROUNDPenile cancer is a rare tumor of male urinary system,and surgery is the main treatment method.The diagnosis and treatment of penile cancer not only seriously affect the physical,psychological and sexual health,but also make men in self-image,self-esteem,social interaction and other aspects have a huge change,and all aspects of each other and interwoven together,so that patients have obvious physiological and psychological dysfunction,at the same time,There are also many psychosocial and emotional care needs for cancer patients in the diagnosis,treatment and follow-up process,and there is little understanding of the unmet psychosocial needs and care needs experienced by men affected by penile cancer.Health education plays an important role in perioperative nursing of patients with penile cancer.However,due to the lack of feedback and evaluation of information,traditional health education usually fails to achieve the expected effect,thus affecting the recovery of patients.Few studies have been conducted on patients with penile cancer in the past,so this study conducted a qualitative study on the experience of patients with penile cancer.Then,According to the results of the study and the problems in clinical nursing,the teach-back method was applied to the perioperative nursing of patients with penile cancer,and the effect of improving the patients’ discharge readiness,improving the patients’ anxiety and improving the patients’ self-efficacy was discussed.OBJECTIVE1.A qualitative study on the experience of penile cancer patients was conducted to provide reference for the formulation of effective nursing interventions2.To evaluate the effectiveness of teach-back in improving patients’ discharge readiness,relieving anxiety,improving self-efficacy and compliance,and to provide a basis for the formulation of perioperative health education strategies for patients with penile cancer.METHODS1.A qualitative study of the experience of illnesses among penile cancer patientsTaking the social ecosystem theory as the theoretical framework,the interview outline was set up,and 11 patients with penile cancer who were treated in a Grade A cancer hospital in Guangzhou from March 2022 to October 2022 were selected as the interview objects by the objective sampling method.Face-to-face semi-structured interviews were conducted on them and data were collected.Oriented content analysis was used to process and analyze the data.2.The application of teach-back method to discharge readiness of patients with penile cancerThe quasi-experimental study method was adopted.A total of 71 patients meeting the inclusion criteria who were admitted to the Department of Urology in a Grade A cancer hospital in Guangzhou from April to December 2022 were selected as the study objects.They were numbered according to the order of admission and divided into experimental group and control group,with 35 patients in the experimental group and 36 patients in the control group.The control group was given routine health education by responsible nurses to patients and their families during perioperative period,and the experimental group was given perioperative health education to patients and their families by teach-back method.The contents of health education in the two groups were consistent,and the intervention time was from the beginning of patients’ admission to the end of discharge.The scores of discharge readiness scale,state anxiety scale and general self-efficacy scale were compared between the two groups,and the number of days in hospital,the number of discharge with tube,the time from surgery back to the first time to get out of bed,and the incidence of complications during hospitalization were statistically compared between the two groups.RESULTS1.The individual social ecosystem of patients with penile cancer is not optimistic,mainly manifested as a series of unmet needs and negative emotions,family and marriage problems,economic pressure and adverse experience of disease diagnosis and treatment process,the patient’s experience and feelings need to be concerned by all parties and take effective measures to meet the patient’s psychological needs and nursing needs.2.The score of discharge readiness of experimental group(95.74±6.24)was higher than that of control group(89.69±6.19),and the difference between the two groups was statistically significant(P<0.05).The anxiety score of experimental group(54.17±6.88)was lower than that of control group(61.61±5.41),and the the difference between the two groups was statistically significant(P<0.05).The selfefficacy score of experimental group(28.31±3.07)was higher than that of control group(26.64±3.31),and the the difference between the two groups was statistically significant(P<0.05).The first postoperative activity time of the experimental group was earlier than that of the control group,and the the difference between the two groups had statistical significance(P<0.05),but there was no statistical significance between the experimental group and the control group in terms of hospitalization days,number of discharge with tube and incidence of complications(P>0.05)CONCLUSIONpenile cancer causes physical and mental health damage of patients,image and cognitive changes,obvious negative emotions,while the disease also brings heavy economic burden and family crisis,so patients hope to obtain family,society,medical system and other parties support and understanding.It suggests that medical staff should pay attention to patients’ demands and unmet physical,mental and social needs,provide them with disease-related knowledge in time,and develop personalized health guidance and rehabilitation programs.Teach-back method is an effective health education strategy for penile cancer patients,which can significantly improve the level of discharge readiness and self-efficacy of penile cancer patients,reduce the anxiety level of patients,improve the compliance of patients getting out of bed activities and shorten the time of patients getting out of bed for the first time after surgery. |
