Objective:To observed and evaluated the effectiveness and safety of 1927nm thulium laser,1927nm thulium laser was used to treating facial photodamage in Chinese individuals.This clinical trial aim to provide evidence for the use of 1927nm thulium laser in treating facial photodamage and offer a new option for photodamage treatment.Methods:We enrolled volunteers who were diagnosed with moderate to severe photoaging in the dermatology department of the Guangdong Second Provincial General Hospital from February 2022 to August 2022,and 31 photodamage volunteers with Fitzpatrick skin classification Ⅱ-Ⅳ was collected.Finally,27 volunteers(24 females and 3 males)with an average age of 44.41 years(33-64 years old)were enrolled for data analysis.The volunteers underwent three comprehensive facial treatments,with an interval of 1 month between each treatment Before each treatment and 1 month after the third treatment,standardized images and the values of wrinkle,spot,texture and pore were taken by VISIA skin detection device.The values measured by VISIA represent the ranking scores in the same region,age group,and skin type,with higher scores being better and a full score of 100.A high frequency skin ultrasonography(Dermascan C USB)was used to measure melanin index(MI),erythema index(EI),skin rebound time(elasticity)and skin hydration,and the results of skin physiological indices were represented by specific values.Moreover,the epidermal thickness and dermal density on ultrasonography were compared between baseline and one month after all treatment sessions,At baseline and final follow-up,two dermatologists referred to the Global Photodamage Scale(GSP)to score facial photodamage in volunteers.Patient subjective assessment mainly included visual analog scale(VAS)pain scores after each treatment and satisfaction scores at final follow-up.Results:After three treatments,the wrinkle score increased from 72.99±13.14 to 79.15±8.114,the pore score increased from 27.92±17.65 to 30.97±17.19,the MI decreased from 36.73±4.269 to 33.81±2.33,the skin rebound time decreased from 278.6±101.0 to 214.9±29.32,and the epidermal thickness increased from 0.1556±0.0073 to 0.1576±0.0075,and all of these improvements had statistically significant differences(P<0.05).In addition,the wrinkle and melanin index showed statistical differences from baseline after the first treatment,and the pore size,skin rebound time,and epidermal thickness showed statistical differences from baseline after the second treatment.The patient’s GSP score decreased from 3.44±0.5 at baseline to 2.67±0.47(P<0.05).The patient satisfaction responses score was 2.04 ± 0.80 points,with 70%of patients reporting "satisfaction" or "extremely satisfaction".Among them,12(44%)subjects reported that their skin complexion had unproved No significant adverse reactions were observed in all 27 subjects."Conclusions:As a new type of treatment,1927nm thulium laser can improve the symptoms of facial wrinkles and enlarged pores in Chinese patients with photodamage,while enhancing skin elasticity and brightening skin color.The postoperative recovery period is short,and patient satisfaction is high.Through clinical trials and case observations,it has been proven that 1927nm thulium laser treatment for facial photodamage is a safe and effective treatment method...
