Objectives:Vimentin is a kind of protein widely existing in a variety of cells and plays an important role in the interaction between cells and the immune system.The post-translational modification and natural form of vimentin are involved in the pathogenesis of inflammation and many autoimmune diseases,and can be used as a diagnostic and prognostic marker for a variety of diseases.Treponema pallidum contains a weak immunogenic cardiolepin antigen that induces the production of a low titer non-treponema antibody.Damaged host cell cardiolepin antigen may play an important role in inducing the production of high titer non-treponema antibodies during syphilis infection.The expression of vimentin in different cells varies,and the most abundant expression is on the surface of macrophage,which is the ultimate executor of the eradication of spirochaeta in syphilis immunization.Unlike cardiolipin antigen,vimentin antigen only comes from damaged cells and treponema pallidum structure does not contain vimentin antigen.Therefore,the purpose of this study was to explore the significance of serum vimentin antibody detection in syphilis patients at different pathological stages.Methods:All the data of 180 patients diagnosed with syphilis in our hospital from December 2019 to December 2022 were analyzed,with positive control group and normal control group as controls.The positive control group included 5 patients with systemic lupus erythematosus(SLE)and 6 patients with rheumatoid arthritis(RA).enzyme-linked immunosorbent assay(ELISA)was used to detect the Antivimentin antibodies(AVA).The correlation between serum anti-vimentin antibody and main laboratory test results and clinical manifestations of syphilis patients,the diagnostic value of anti-vimentin antibody and the change of the concentration of anti-vimentin antibody before and after treatment in patients with syphilis serum fixation were analyzed.Results:The serum anti-vimentin antibody of syphilis patients was significantly different from that of normal control group,and higher positive rate was detected in peripheral blood of syphilis patients at different stages.2.There was a correlation between serum anti-vimentin antibody expression level and syphilis serological test result(TRUST titer).3.Serum anti-vimentin antibodies have high specificity and sensitivity for the positive rate of syphilis.4.The level of anti-vimentin antibody in patients with syphilis serum fixation after treatment was lower than that before treatment.Conclusion:1.Anti-vimentin antibodies exist in the peripheral blood of patients with syphilis,and the combined detection of the antibodies is of great importance in assisting the clinical diagnosis of syphilis.2.Positive anti-vimentin antibody indicates that immune system damage may be relatively serious,and patients are more prone to genital ulcers and nervous system symptoms.Continuous detection of this index is helpful to evaluate the process of system damage.3.The serum anti-vimentin antibody level of patients with syphilis serologic fixation decreased after receiving standard treatment.
