Objective:On the basis of the theory of "combination of motion and static",compare the two different operation methods of Waveflex hybrid semi-rigid internal fixation system and total laminectomy and nucleus pulposus removal,decompression,interbody fusion and rigid internal fixation system,respectively observe the postoperative lumbar stability,range of motion,the degree of intervertebral disc degeneration of adjacent segments of patients with two different operation methods,and conduct follow-up survey of patients,Comprehensive use of statistical methods to study and draw conclusions,so as to provide a reference basis for doctors’ clinical diagnosis and treatment.Methods: Collect and screen 60 patients with two-stage lumbar degenerative disease admitted to the Department of Spinal Orthopedics,Affiliated Hospital of Shandong University of Traditional Chinese Medicine from October 2019 to October2022.Two groups of patients were randomly divided into two groups with two different surgical schemes: one group was treated with double-segment total laminectomy,decompression,nucleus pulposus extraction,interbody fusion and internal fixation,30 cases were treated as fusion group;In the other group,30 cases were treated with Waveflex hybrid semi-rigid internal fixation.Information was collected from 60 patients: 1.Basic data,including age,sex and course of disease;2.intraoperative data,including intraoperative operation time and bleeding volume;3.Scale evaluation data,including: Japanese Orthopaedic Association Scores(JOA),Oswestry Disability Index(ODI)and Visual Analogue Scale(VAS)of pain before operation,3 months after operation,6 months after operation and at the last follow-up;4.Imaging data,including the intervertebral space height index(IHI)and intervertebral range of motion(ROM)of adjacent segments before the operation and at the last follow-up,as well as the evaluation of the change of intervertebral disc using the Pfirmann grading and the evaluation of the degenerative change of the facet joint using the Weisaupt grading.Through the statistics of the above data,the clinical effects of two different surgical methods in the short and medium term were compared.Results: 1.After statistical analysis and comparison of the basic data of the two groups of patients,no significant difference was found(P>0.05);There was no significant difference between the two groups in intraoperative bleeding volume and bleeding volume(P>0.05);2.At the last follow-up,there was a significant difference in VAS scores between the two groups(P<0.05),but there was no significant difference in VAS scores at 3 and 6months after operation(P>0.05),and there was no significant difference in Oswestry dysfunction index and JOA scores at different time points(P>0.05).In the two groups,the VAS score,Oswestry dysfunction index and JOA score of patients at the last follow-up were significantly different from those before surgery(P<0.05).3.In terms of imaging indicators,there was a statistically significant difference between IHI and ROM of the two groups at the last follow-up and preoperative time point(P<0.05).There was a statistically significant difference in ROM between the two groups at the last follow-up(P<0.05).There was no statistically significant difference in IHI between the two groups at the last follow-up(P>0.05).In Weisaupt grading,20 cases(66.7%)in the semi-rigid group did not change,10 cases(33.3%)had degeneration,16 cases(53.3%)in the fusion group did not change,and 14 cases(46.7%)had degeneration.There was no statistically significant difference between the groups(P>0.05).In the Pfirmann grading,22 cases(73.3%)in the semi-rigid group did not change,8 cases(26.7%)had degeneration,and 14 cases(46.7%)did not change in the fusion group,There were 16 cases(53.3%)with degeneration,and the difference between groups was statistically significant(P<0.05).Conclusion:1.There is no significant difference between traditional fusion surgery and semi-rigid internal fixation surgery in operation time,intraoperative bleeding volume and surgical injury2.Both groups have therapeutic effects on lumbar degenerative diseases.The short-term clinical effects of the two surgical methods are similar.The medium-term pain control effect of semi-rigid fixation is better than that of fusion fixation.3.Semi-rigid fixation can delay the degeneration of adjacent intervertebral discs.4.The Waveflex hybrid semi-rigid fixation surgery based on the guidance of the "combination of motion and static" theory can effectively alleviate the symptoms of patients,and has satisfactory performance in delaying the degeneration of adjacent segments of intervertebral disc and the evaluation of patient’s score,which is worth promoting.
