The family doctor contract system is a reasonable choice for deepening the reform of basic health services in urban and rural areas in China,which is of great significance to"primary diagnosis","health equity" and "comprehensive health",and has positive significance for coping with changes in people’s health needs,improving the accessibility of primary health services,and ensuring the effective use of primary health resources.Family doctor contracting is an emerging medical management model,which has a special value orientation and a foundation for theory,policy and practice.However,due to the limitations of many factors,the implementation of this policy still faces many practical problems,and in order to achieve its due purpose,it is also necessary to optimize and improve the problems existing in actual operation.After the implementation of family doctor contracting in T area,it has explored many aspects,including the construction of a family doctor team and a family doctor contracting system,and although some achievements have been made,there are still many problems.These problems are not only closely related to people’s physical health,but also have many impacts on the in-depth development of medical and health work in T area.Therefore,at present,it is urgent to solve the outstanding problems in the contracting of family doctors,and formulate a sound implementation method for the family doctor contracting policy for the T zone.Based on the Massa Comprehensive Model,this study constructs an analytical framework for the implementation and optimization of family doctor signing service policies.It uses research methods such as questionnaire surveys and interviews to collect relevant data and analyze the current situation,problems,and reasons for the implementation of family doctor signing service policies in the T-zone.The study found that the implementation plan for the contract service policy for family doctors in T District is to establish the implementation goal of achieving full coverage of the contract,refine the implementation content,promote the implementation of the policy in place,innovate the implementation methods,and improve the service policy implementation organization.Specific implementation measures include:clarifying the coordination of multiple entities to promote policy implementation,ensuring policy implementation through policy resource allocation,promoting policy awareness among target groups,and monitoring and evaluation mechanisms to ensure policy modification.The main problems in the implementation of this policy include:a large gap between the implementation effect and expectations of the policy,low awareness of residents’ policies,insufficient promotion and education of family doctor services,low satisfaction of residents with contracted services,and lack of tracking management of contracted services.Through analysis,it is found that the main influencing factors for these problems include:insufficient integration of policy formulation with the actual situation in the region,relatively single contract service content,insufficient information channels,low job satisfaction of family doctors,imperfect training and incentive mechanisms for family doctors,low professional identity of family doctors,and imperfect policy supervision system.On this basis,suggestions for optimizing family doctor signing service policies are proposed,including:improving the signing service policy itself,enhancing the support of signing service policy resources,improving the executive power of policy implementation organizations,strengthening the supervision of signing service policies,and constructing a good policy implementation environment.
