Objective To construct a set of systematic,perfect,scientific and in line with national conditions of the perioperative pain management of prostate puncture biopsy,so as to provide reference for the pain management of clinical patients undergoing prostate puncture biopsy,so as to reduce their perioperative pain,improve the comfort level of patients,help increase the positive detection rate of samples,and reduce postoperative complications.Method Based on the literature review,semi-structured interviews were conducted using purposive sampling method with 10 prostate puncture patients to explore the feelings and needs of clinical prostate puncture biopsy patients’ pain management;15 medical and nursing staff from the Department of Urology of Shaoxing People’s Hospital were selected for qualitative interviews to understand the current situation of clinical staff on pain management of prostate puncture biopsy patients.The literature analysis method was used to systematically retrieve the literature related to pain management of prostate puncture biopsy and then evaluate and grade the quality of the included literature based on PET model,establish a program framework,and initially form a peripuncture pain management program for patients undergoing prostate puncture biopsy.The final draft of the peri-penetrating pain management protocol for patients undergoing prostate puncture biopsy was formed according to the Delphi modification protocol.Result ①Qualitative interviews were conducted in sequence between the patient group and the medical staff group.According to the interview data,three aspects of pain management needs of prostate biopsy patients were condensed:The needs of knowledge,pain management skills and physiological needs,while clinical pain management has three aspects of low attention,lack of knowledge and obstacle factors,which reflects the clinical needs and improvement direction of the construction of prostate puncture biopsy pain management program.②This study screened literatures based on PET model,and 23 literatures were included;It was classified according to the literature type,including 1 systematic review,21 RCTs,and 1 expert group consensus.According to the classification of quality grade,the final included papers included Ⅰ/B 22,Ⅳ/A1.After analysis and summary,three aspects including pre-operation,during operation and after operation were extracted to form a preliminary pain management program for patients undergoing prostate biopsy in the peripuncture period.③This pain management protocol involved three interventions before,during,and after the operation,covering the pain management of patients throughout the peripuncture period.After two rounds of expert Delphi correspondence,this study resulted in a pain management protocol for prostate puncture biopsy that included 3 primary entries,21 secondary entries,and 34 tertiary entries.Conclusion Based on evidence-based care and the Delphi method,this study established a pain management program for prostate puncture biopsy based on the steps of standardized research,scientific and reliable methods,the needs and feelings of patients,and the actual clinical situation,providing a theoretical basis and guiding direction for the medical treatment of prostate puncture biopsy,with a view to reducing the level of intraoperative and postoperative pain,thereby improving the anxiety level of patients,reducing the incidence of vagal reflexes during puncture,and reducing postoperative complications,with strong relevance and feasibility.
