| Objective The Eustachian tube function in children was evaluated by the new Eustachian tube function scoring scheme T-ETDQ(Tubomanometry Eustachian Tube Dysfunciton Questionnaire),and compared with the ETS(Eustachian tube score)scheme to explore the diagnostic value and clinical application potential of T-ETDQ for Eustachian tube dysfunction in children.Methods This prospective diagnostic study included patients visiting the ENT Department of Tongde Hospital of Zhejiang Province between February 2022 and December 2022,and the patients were divided into abnormal and normal groups according to the results of the tympanometrym.1.Compare the baseline data between the two groups(gender,age,the results of T-ETDQ and ETS scores).2.Analyze the main influencing factors in the diagnosis of Eustachian tube dysfunction.3.Analyze and evaluate the diagnostic value of T-ETDQ and ETS by the receiver operating characteristic(ROC)curve,and determine the optimal positive cut-off for T-ETDQ and ETS scores in children subjects.Then,re-judge the results of the subject’s T-ETDQ and ETS score.4.Compare the results of the redetermined T-ETDQ and ETS scheme with the tympanometrym,to explore the value of T-ETDQ in assessing Eustachian tube dysfunction in children.Results There were 45 subjects(60 ears)in the abnormal group,including 22 men(30ears)and 23 women(30 ears),ranging age 6~17 years of age,with an average age of 12.62±2.93 years.There were 27 patients(31 ears)in the normal group,including 12 men(14 ears)and 15 women(17 ears),ranging age 7~17 years of age,with an average age of 13.03±2.79 years.1.There were no statistically significant differences in age or sex between the two groups(P>0.05),and there were statistically significant differences between ETS and T-ETDQ scores in both groups(P<0.001).2.The results of logistic regression analysis showed that T-ETDQ(OR=8.068,95%CI:2.501~26.029)had a significant statistical significance in the diagnosis of Eustachian tube dysfunction(p<0.01),and had a more significant impact than ETS.3.The area under the ROC curve(AUC)of T-ETDQ was 0.973(95%CI:0.945~1.000),and the AUC of ETS was 0.872(95%CI:0.797~0.947).The highest sensitivity and specifcity of T-ETDQ were 95.0%and 90.3%respectively when the positive cut-of value was 3.93.And the highest sensitivity and specifcity of ETS were 80.0%and 80.6%respectively when the positive cut-of value was 5.5.4.The kappa values obtained by comparing the redetermined ETS and T-ETDQ results with the tympanometrym were 0.579 and 0.853,respectively(P<0.001 for both).And the results showed that T-ETDQ was more consistent with the tympanogram than ETS.Conclusion T-ETDQ has high consistency with the tympanometrym,can efectively quantify and evaluate ETD severity in children,has a higher diagnostic value than ETS,making it a suitable diagnostic tool. |
