Objective:The aim of this clinical study is to compare the clinical outcomes of complete mesocolon resection(CME)in the treatment of colon cancer patients through open surgery and laparoscopic surgery,and to clarify the value and advantages of laparoscopic complete mesocolon resection in the surgical treatment of colon cancer.Methods:The study selected 80 patients with colon cancer who were admitted to the Gastrointestinal Surgery Department of the Affiliated Hospital of Jining Medical University and operated on by the same surgeon in the same medical team from August 2017 to September 2019.According to the different surgical methods,these patients were divided into the open group and the endoscopic group.Then 38 patients in the open group received complete mesocolon resection under open surgery,while 42 patients in the endoscopic group received complete mesocolon resection under laparoscopy.The surgical related indicators(intraoperative bleeding,number of lymph node dissection,surgical time,recovery time of postoperative bowel sounds,time of first postoperative exhaust,frequency of use of painkillers within 48 hours),occurrence of common postoperative complications(postoperative incision infection,anastomotic fistula,anastomotic stenosis,intestinal obstruction,incision dehiscence,respiratory tract infection,ureteral injury),the expression of the inflammatory factors(CRP and TNF-α)preoperative and postoperative and changes in tumor markers(carcinoembryonic antigen CEA)were evaluated.Results:Compared with patients in the open group,the number of intraoperative lymph node dissection in the endoscopic group was significantly increased,the recovery time of bowel sounds and the first exhaust time were shortened,and the frequency of analgesic drugs(tramadol 100mg/intramuscular injection)used within 48 hours was reduced,the difference was statistically significant(P<0.05).However,there was no significant difference between intraoperative bleeding volume and operative time(P>0.05).There was no statistically significant difference between the two groups in postoperative complications(incision infection,anastomotic fistula,anastomotic stenosis,intestinal obstruction,respiratory tract infection,ureteral injury,and incision dehiscence)(P>0.05).Compared with the preoperative period,the expression of inflammatory factors(CRP and TNF-α)was increased in the fasting venous blood at 3 days after surgery.Further more,the increase of inflammatory factors in the endoscopic group was significantly lower than that in the open group,the difference was statistically significant(P<0.05).And the expression of carcinoembryonic antigen in postoperative patients was significantly reduced in the open group.The difference was statistically significant(P<0.05).So was the endoscopic group.Conclusion:On the premise of treating colon cancer by complete mesocolic resection,compared with traditional open surgery,laparoscopic surgery can clear the lymph nodes in the operation area more thoroughly,reduce the postoperative traumatic stress to a greater extent,and promote the recovery of postoperative intestinal function at an earlier stage without affecting the incidence of postoperative complications,Therefore the laparoscopic surgery is beneficial to the rapid recovery and long-term prognosis of patients,and has greater clinical promotion value.
