| Objective:Selvester QRS scoring system is an effective method to evaluate myocardium scar area using electrocardiogram indicators.However,there are few reports on the evaluation of left heart function in DCM patients,and the relationship between it and peripheral blood NT-proBNP,cTnl and myocardial enzyme profile indicators is not clear.The purpose of this study was to investigate the relationship between Selvester QRS score and left heart function,NT-proBNP,troponin and myocardial enzyme profile in patients with dilated cardiomyopathy.Methods:A total of 24 DCM patients treated in the First People’s Hospital of Jining City from January 2021 to December 2022 were selected as the case group,and 20 healthy physical examination patients from the hospital were selected as the control group during the same period.Cardiac magnetic resonance examination was performed,and left ventricular function parameters of each group were measured:LVEF,LVEDV,LVESV,LVSV,LVCO and LVMM,and peripheral venous blood was extracted to detect myocardial enzyme profile indexes:CK and CK-MB,while cTnI and NT-proBNP indexes were detected.Standard 12-lead electrocardiogram was performed and Selvester QRS score was calculated.24 DCM patients were divided into negative group(score=0)and positive group(score≥1)according to the specific score.Combined with the results of CMR and peripheral blood examination,the differences of left cardiac function parameters among DCM group,negative group,positive group and control group,as well as the differences of NT-proBNP,cTnI and myocardial enzyme profile indexes between negative group and positive group were compared.The correlation between Selvester QRS score and left cardiac function parameters,NT-proBNP,cTnI and myocardial enzyme profile was analyzed.Results:(1)There were no significant differences in age,gender,BMI and heart rate between the DCM group and the control group in general data(P>0.05).(2)Comparison of CMR parameters of left ventricular function among DCM group,negative group,positive group and control group:Compared with control group,LVEDV,LVESV and LVMM in DCM group were significantly higher than those in control group(P<0.05),while LVEF,LVSV and LVCO in DCM group were significantly lower than those in control group(P<0.05).LVESV of negative group was significantly higher than that of control group(P<0.05),while LVEF and LVSV of negative group were significantly lower than that of control group(P<0.05).LVEDV,LVESV and LVMM in positive group were significantly higher than those in control group(P<0.05),while LVEF,LVSV and LVCO in positive group were significantly lower than those in control group(P<0.05).The comparison of left ventricular function parameters between negative and positive groups showed that LVEDV,LVESV and LVMM in positive group were significantly higher than those in negative group(P<0.05),while LVEF was significantly lower than that in negative group(P<0.05).(3)Compared with the negative group,the value of NT-proBNP in the positive group was higher than that in the negative group,and the difference between the two groups was statistically significant(P<0.05),while the differences in cTnI,CK and CK-MB were not statistically significant(P>0.05).(4)Correlation analysis showed that in the positive group,Selvester QRS score was negatively correlated with LVEF,LVCO and CK(rs=-0.527,-0.66 and-0.584,P=0.016,0.004 and 0.014).It was positively correlated with LVEDV,LVESV,NT-proBNP and cTnI(rs=0.858,0.642,0.762 and 0.615,P<0.001,0.005,<0.001 and 0.009).Conclusion:1.Selvester QRS score can be used as an electrocardiogram to evaluate left heart function in patients with DCM.2.Selvester QRS score can provide new ideas for the process of ventricular remodeling in DCM.3.Selvester QRS score was significantly positively correlated with NT-proBNP level. |
