| Objective:To investigate the clinical application of digital positioning guide plate in extraction of complete bone embedded teeth in maxillary palatine anterior.Methods:A total of 32 patients with complete bone embedded teeth in maxillary palatine treated in the oral and maxillofacial surgery ward of the Affiliated Hospital of North Sichuan Medical College from November 2020 to December 2021 were selected as the study subjects.They were randomly divided into experimental group(with digital positioning guide plate)and control group(with routine operation),16 patients in each group.The exposure time of embedded teeth,operation time,intraoperative blood loss,intraoperative and postoperative complications were recorded in each group.Results:(1)A total of 16 cases were included in the experimental group(using the digital positioning guide plate),including 7 females and 9 males in the experimental group,with an average age of 7.1±1.7 years(5-10 years old);a total of 16 maxillary anterior teeth on the palatal area completely bone embedded teeth,15 of which were embedded supernumerary teeth and 1 maxillary central incisor(21)were impacted upside down.A total of 16 patients in the control group(conventional surgery)were included,including 6 females and 10 males,with an average age of 7.5±1.9 years(4-11 years old);a total of 16 completely buried teeth on the palatal side of the maxillary anterior region,of which 15 were embedded supernumerary teeth,1 maxillary canine(13)was ambush upside down.There were no significant differences in gender and age between the two groups(P>0.05).(2)The exposure time of the embedded teeth in the experimental group was(3.85±0.70)min,which was significantly shorter than that in the control group[(6.51±2.82)min],and the difference between the two groups was statistically significant(t=3.66 P=0.002);the experimental group underwent surgery The time was(18.63±2.83)min,less than the control group[(25.83±4.67)min],the difference between the two groups was statistically significant(t=5.275 P=0.000);the intraoperative blood loss in the experimental group was(9.47±1.76)ml,lower than the control group[(12.27±2.53)ml],the difference between the two groups was statistically significant(t=3.63 P=0.001).(3)There were no intraoperative and postoperative complications such as adjacent tooth injury,nerve injury,nasal perforation,gingival tear,infection,etc.in the two groups.1 patient in the control group,there was local oozing at the gingival margin in the area 2 hours after the operation,and no obvious oozing occurred after 20 minutes of local gauze occlusion and pressure to stop the bleeding.All patients in the experimental group used the positioning guide plate during the operation,and there was no positioning deviation,and the position of the embedded tooth was consistent with the positioning position of the guide plate.Conclusion:The digital positioning guide plate can accurately transfer the image data of preoperative cone beam CT to locate the position of embedded teeth and the surgical approach designed before surgery to the operation area,and can quickly and accurately locate the maxillary embedded teeth during the operation.It can shorten the operation time,reduce the amount of bleeding,reduce the difficulty of operation,improve the quality of operation,and make the operation more precise and less invasive.It has a good application prospect and is worthy of clinical promotion. |
