Objective:To investigate the expression of myeloid cell leukemia-1(MCL-1)protein and BRAF V600E(VE1)protein in colorectal cancer(CRC)patients,and its correlation with clinicopathological features.The correlation between the expression of MCL-1 protein and VE1 protein in CRC and prognosis of patients is analyzed too.Method:Immunohistochemistry(IHC)was used to detect the expression of MCL-1 protein and BRAF V600E(VE1)protein with paraffin sections of radical resection specimen from 207 cases of CRC patients admitted to The First People’s Hospital of ZiGong from January 2015 to December 2016.The enrolled patients were followed up for at least 5 years,including postoperative treatment,tumor metastasis,tumor recurrence and patient survival.The relationship between the expression of MCL-1 and VE1 protein and clinicopathological indicators such as gender,age,tumor site,histological grade,depth of invasion,vascular invasion,nerve invasion and lymph node metastasis of CRC patients was analyzed.The correlation between McL-1 protein expression and VE1 protein expression was analyzed.The relationship between MCL-1 protein,VE1 protein and prognosis of CRC patients was analyzed too.χ2 test of composition ratio of two samples(Fisher’s exact probability method was used when the sample size was less than 40 cases)was used to compare the rates.Correlation analysis was performed with Chi-square test four-fold table method.Kaplan-meier survival analysis and log-rank test were used for univariate analysis,and Cox regression was used for multivariate analysis."P<0.05”was regarded as the judgment standard for statistically significant difference.All data were analyzed with SPSS 25.0 statistical software.Result:1.The positive expression rate of MCL-1 protein in CRC was 75.4%(156/207).The positive signal was brownish or brownish yellow and mainly located in cytoplasm or nucleus.The positive expression rate of BRAF V600E protein in CRC was 4.8%(10/207).The positive signal was light yellow and mainly located in the cytoplasm.2.The positive expression rate of MCL-1 protein in rectal cancer and colon cancer was 81.8%(81/99)and 69.4%(75/108)respectively.The positive expression rate of MCL-1 protein in rectal cancer was significantly higher than that in colon cancer(P=0.039)and the difference was statistically significant.However,MCL-1 protein expression was not correlated with gender,age,histologic grading,depth of tumor invasion,vascular invasion,nerve invasion and lymph node metastasis(P>0.05).BRAF V600E protein expression was not correlated with gender,age,tumor site,histologic grading,depth of tumor invasion,vascular invasion,nerve invasion and lymph node metastasis too(P>0.05).3.The number of positive expression cases of MCL-1 and BRAF V600E protein in CRC was 156 and 10,respectively.9 cases were positive expression of both MCL-1 and BRAF V600E protein,however 50 cases were negative.Correlation analysis showed that there was no correlation between MCL-1 protein and BRAF V600E protein expression in CRC patients(χ2=0.526,P>0.05).4.A total of 207 patients were followed up until September 30,2021.Following up data was gotten from 114 patients(114/207=55.1%)and an average follow-up time was 67.9 months.Among them,42 patients were dead and 72 patients were survival.The mean survival time was 53.0 months and the median survival time was 61.0 months.The mean survival time of MCL-1 positive patients was 50.4months,and that of MCL-1 negative patients was 60.1 months.The median survival time of MCL-1 positive patients was 59 months,and that of MCL-1 negative patients was 62.5 months.The five-year survival rate was 58.3%in MCL-1 positive patients and 80%in MCL-1 negative patients.The mean survival time,median survival time and 5-year survival rate of CRC patients with MCL-1 positive expression were lower than those with MCL-1 negative expression,and the prognosis was worse(P<0.05).5.The survival rate of CRC patients whose tumor invaded the muscle layer and the full-thickness of intestinal wall was 84.2%(16/19)and 58.9%(56/95),respectively.The survival rate of CRC patients whose tumor only invaded the muscle layer of the intestinal wall was significantly higher than that of patients whose tumor invaded the full-thickness of intestinal wall(P=0.049).The survival rate of CRC patients with and without nerve invasion was 30.0%(3/10)and 66.3%(69/104),respectively.The survival rate of CRC patients without nerve invasion was significantly higher than that of CRC patients with nerve invasion(P=0.004).Conclusion:1.The positive expression rate of MCL-1 protein in rectal cancer was significantly higher than that in colon cancer,but there was no significant difference in prognosis.2.There was no correlation between MCL-1 protein expression and BRAF V600E protein expression in CRC.3.CRC patients with full-thickness invasion of intestinal wall,nerve invasion and positive expression of MCL-1 had a worse prognosis and lower five-year survival rate.It suggested that the presence of neurological invasion is an independent prognostic factor in CRC patients.4.There was no correlation between BRAF V600E protein expression and prognosis of CRC patients. |
