| Objective:To investigate the prescription prescription of patients with cirrhosis for the first time,and to provide some reference for clinical drug prescription.To find the target information and possible related signal pathways of core drugs in the treatment of patients with cirrhosis who were first diagnosed clinically,so as to lay a theoretical foundation for further basic experimental research.Methods:1.Collected the case data and TCM prescriptions of 193 patients who first visited the Department of Liver Disease,TCM Combined Hospital of Southern Medical University from January 2014 to December 2021.SPSS Statistic software was used to conduct frequency statistics on the general information,TCM syndromes and TCM of the patients.SPSS Modeler software was used for cluster analysis and association rule analysis of TCM prescriptions.2.Combined with the data mining results,extract the core drugs,screen the active components and action targets of traditional Chinese medicine by tcmsp,retrieve the disease targets by genecards and other databases,and construct the "disease drug component target"network.Take the intersection of the action target of traditional Chinese medicine and the disease target to obtain the common action target.Import common targets into string platform,build PPI network and obtain key targets.Go enrichment analysis and KEGG pathway analysis were carried out by R software.Results:1.A total of 193 clinical cases were included in this study,including 144 male patients and 49 female patients.The age of onset is mainly 45~59 years old,with 92 cases.HBV is the main cause of liver cirrhosis,with 133 cases.The main complication was ascites,with 89 cases.Six syndrome types are summarized,which are ranked from high to low in frequency,followed by qi stagnation and dampness obstruction syndrome,damp heat accumulating spleen syndrome,liver and kidney yin deficiency syndrome,liver and spleen blood stasis syndrome,spleen and kidney yang deficiency syndrome and cold dampness trapping spleen syndrome.The 193 traditional Chinese medicine prescriptions contain 162 traditional Chinese medicines,and the cumulative use frequency of all traditional Chinese medicines is 2341 times.The traditional Chinese medicines are bupleurum,Poria cocos,licorice,Atractylodes macrocephala,astragalus,tangerine peel,Alisma orientalis,white peony,dangshen and Fructus aurantii.Cluster analysis can be divided into five categories of traditional Chinese medicine prescriptions,namely "big belly skin,radish seed,turtle shell,Polyporus umbellatus,prepared rehmannia glutinosa,turmeric,Alisma orientalis,Rehmannia glutinosa,peony bark,Fructus aurantii,Magnolia officinalis","gardenia,rhubarb,Scutellaria baicalensis,Herba capillaris","yam,coix seed,dangshen,Pinellia ternata,Atractylodes macrocephala,Poria cocos,astragalus,tangerine peel,hawthorn" and "angelica,Ophiopogon japonicus,white peony","bupleurum,licorice,Ligusticum chuanxiong".Using correlation analysis and complex network analysis,the core drug group with the best correlation degree was selected as "bupleurum,Poria cocos and Atractylodes macrocephala".2.Through the network pharmacological analysis of core drugs,it was found that there were 39 active components such as quercetin,baicalin,yinchenflavone,isorhamnetin and kaempferol.The core targets were Jun,AKT1,IL6,myc and Mapkl4.The signal pathways included AGE-RAGE signal pathway,TNF signal pathway,HIF-1 signal pathway,IL-17 signal pathway,relaxin signal pathway,toll like receptor signal pathway and C-type lectin receptor signal pathway.Conclusions:1.Data mining:The core drugs for the treatment of liver cirrhosis are "bupleurum,Atractylodes macrocephala and poria cocos",suggesting that traditional Chinese medicine is mainly used to treat liver cirrhosis in the liver and spleen,and the main treatment methods are soothing the liver,strengthening the spleen and promoting water and dampness.2.Network pharmacology:The core drugs for the treatment of liver cirrhosis may be completed by interfering with target genes such as Jun,AKT1 and IL6,antioxidant stress and signaling pathways such as AGE-RAGE. |
