Objective:malnutrition is common in patients with esophageal and gastric fundus varices due to cirrhosis.After endoscopic treatment,the nutritional problems of patients are more prominent.Nutritional intervention should be given to patients as soon as possible to improve the prognosis.However,there is no clear guideline and consensus on which nutritional preparation is more suitable for such patients.This study aims to apply two different enteral nutritional preparations,short peptide type and integrin type,to nutritional intervention for such patients,The effectiveness and safety of the two preparations were further analyzed to provide a theoretical basis for clinical treatment.Methods:According to inclusion and exclusion criteria,90 patients with hepatitis B virus-related cirrhosis and esophagogastric varices requiring endoscopic treatment were selected from Tai’an central hospital.The patients were divided into three groups with 30 cases in each group by random number table method.All patients in the three groups were given symptomatic supportive treatment such as hemostasis,acid inhibition and enzyme inhibition after endoscopic treatment.Group A(whole protein group)was treated with parenteral nutrition+whole protein enteral nutrition+general liquid diet;Group B(short peptide group)was treated with parenteral nutrition+short peptide enteral nutrition+general liquid diet;Group C(general diet group)was treated with parenteral nutrition+general liquid diet.Nutritional risk screening score(NRS 2002),body mass index(BMI),biochemical index,inflammatory index and gastrointestinal reaction of the three groups of patients were compared before treatment and 7 days after treatment.Results:(1)There was no difference in the general data of the three groups,such as age,sex,educational level,etiology,liver function grade and other indicators(P>0.05)(2)There was no difference in NRS2002 score,BMI,biochemical index and inflammatory index among the three groups before treatment(P>0.05).(3)After treatment,the NRS2002 scores of the three groups were significantly different(P<0.05),and the whole protein group was better than the short peptide group and the general diet group,there was significant difference among three groups.(4)There was no significant difference in BMI among the three groups after treatment(P>0.05),and the body weight of the three groups after treatment was slightly lower than that before.(5)After intervention,the serum prealbumin level of the whole protein group was better than that of the short peptide group and the general diet group,and the difference was statistically significant(P<0.05).The effects of the whole protein group on serum total protein level,serum albumin level and fasting blood glucose were better than those of the short peptide group and the general diet group.(6)The level of interleukin-6(IL-6)in the whole protein group was significantly decreased after intervention,while the results of the other two groups were opposite,the difference was statistically significant(P<0.05).(7)Compared with the other two groups,the nausea and diarrhea rates in the short peptide group were significantly higher,and the differences were statistically significant(P<0.05).Conclusion:For patients with esophageal and gastric fundus varices of cirrhosis treated by endoscopy;Whole protein enteral nutrition preparation has better effect on improving nutritional status of patients than short peptide enteral nutrition preparation,and has lower gastrointestinal side reactions than short peptide enteral nutrition preparation.
