| Objective The purpose of this study is to understand the cognitive status of early activity in elderly frail patients with hip replacement under the concept of accelerated rehabilitation,to explore the psychological and physiological experience of early activity in elderly frail patients after hip replacement,to analyze the motivation and influencing factors of early activity,and to provide theoretical basis for medical staff to formulate early activity plan for elderly frail patients with artificial hip replacement.Methods Using the method of descriptive qualitative research,the patients who underwent hip replacement in the Department of Orthopedics and Traumatology of Grade A Level Three hospitals in Hangzhou,were selected according to the maximum variation strategy through purpose sampling.Participatory unstructured observation and face-to-face semi-structured interviews were used to collect data.The data were analyzed according to the conventioanl content analysis method,and the themes were formed through repeated coding and classification,until the information was saturated.Results A total of 16 elderly frail patients undergoing hip replacement were included as the research objects.After sorting out and analyzing the data,the following five themes and sixteen sub themes were formed:1.Cognition of early activities:early activities are necessary,and early activities should be "slow";2.Motivation of early activities:meet the general needs of life,reduce the burden of family care,and desire to return to the family;3.Concerns about early activities:the body cannot tolerate,worry about "safety",and the concern of past falls;4.Early activity promoting factors:mastering rehabilitation knowledge,active guidance of rehabilitation personnel,a sense of security from rehabilitation equipment,rehabilitation exercise self-efficacy;5.Hindering factors of early activity:uninformed surgery,early unsupervised guidance,lack of quantitative indicators,and bad emotional interference.Conclusion This study found that elderly frail patients with hip replacement have insufficient understanding of early activity.Patients recognize and accept early activity.However,due to the concerns of their own frail physique,patients propose that early activity should pay attention to methods and individual differences.Patients have safety concerns during early activities and show a cautious attitude.Through further analysis,it is found that patient selfefficacy,rehabilitation knowledge,and rehabilitation medical personnel and equipment are the influencing factors of early activities.This study suggests that,first of all,medical staff should improve the understanding of the disease in elderly patients with frailty,and eliminate patients’concerns,improve patients’self-efficacy.It is also necessary to strengthen the training of medical staff to provide rehabilitation resources to build a good hip replacement support system for elderly patients with frailty,so that accelerated rehabilitation concept can really play a role. |
