| Objectives:(1)To analyze the cervical cancer screening situation of Qinghai region in the ten years from 2011 to 2020;and(2)the change situation of cervical HPV(human papilloma virus)infection types in the ten years,thus providing individualized and accurate solutions for prevention and cure of cervical cancer.Methods:The HPV screening results of women who visited the gynaecological clinic or inpatient department or had a physical examination in the Affiliated Hospital of Qinghai University,the People’s Hospital of Qinghai Province and the Red Cross Hospital of Qinghai Province from January 2011 to December 2020 were collected.Statistics were performed on the distribution of different HPV subtypes in these women in different years and in the ten years,the changes of each type were analyzed,and the distribution of HPV positivity in different nationalities and different age groups was compared.Results: A total of 76,511 pieces of data that met the inclusion criteria were collected,of which the number of HPV-positive cases was 12,980,with the positive rate of 16.96%.According to age groups,those aged 36-55 accounted for over 60% of the total,and those aged >65 had a small quantity.Those aged ≤65 had the same top four HPV infection types,namely HPV 16,58,52 and 39.In addition to HPV 16 and 58,those aged >65 also had the infection types of HPV 68 and 31,which was different from those aged ≤65.The top nine HPV infection types in the ten years from 2011 to 2020 were HPV 16,58,52,39,18,53,68,51 and 31,of which HPV 16 always ranked first in each year,HPV 58 ranked second in each year except 2018(HPV 52 ranked second in the two years),and those ranked third to fifth were different in different years.By comparing various HPV types among the Han,Zang and Hui nationalities,it could be known that there was a statistically significant difference in the six types of HPV 16,18,31,33,45 and 52.By analyzing each type,it could be seen that there was a statistically significant difference in HPV 16 between the Han and Zang nationalities as well as the Han and Hui nationalities,but there was no statistically significant difference between the Hui and Zang nationalities;there were no statistically significant differences in HPV 18 between any two groups of the Han,Zang and Hui nationalities;there was a statistically significant difference in HPV 31 between the Hui and Zang nationalities as well as the Han and Zang nationalities,but there was no statistically significant difference between the Han and Hui nationalities;and there was a statistically significant difference in HPV 33,45 and 52 between the Han and Hui nationalities as well as the Zang and Hui nationalities,but there was no statistically significant difference between the Han and Zang nationalities.Conclusions:(1)The HPV positive rate of Qinghai region in the ten years from 2011 to 2020 shows a rising trend,of which the HPV positive rate in 2018 ranks the highest.The top five infection subtypes are HPV 16,58,52,39 and 18.HPV 16 always ranks first in each year,and HPV 58 ranks second in each year except 2018(HPV 52 ranks second in 2018).Therefore,besides the population with HPV 16 positivity,special attention should also be paid to those with HPV 58 positivity in Qinghai region.(2)The HPV positive rate gradually increases as the age increases.Although those aged >65 have the minimum quantity for screening,they have a high HPV positive rate.Therefore,intensive screening and the publicity of tertiary prevention from cervical cancer should be carried out to women aged >65.(3)HPV infection types are different in different nationalities.HPV 31 has the highest positive rate in the Zang nationality except HPV 16;the positive rate of HPV 33,45 and 52 in the Han and Zang nationalities is apparently higher than that in the Hui nationality.Therefore,for the Zang population,in addition to HPV 16,attention should also be paid to HPV 31,33,45 and 52,especially to those with HPV 31 positivity. |
