| Objection: The present situation of reproductive health of female workers in petrochemical industry was investigated and the influencing factors of reproductive health of female workers in petrochemical industry were analyzed.The establishment of the rosette risk model for three reproductive health problems(abnormal menstruation,reproductive tract infection,perimenopausal syndrome),the prediction of risk,visual evaluation and analysis of the influencing factors,is of great significance to improve the health of female petrochemical workers and the improvement of health measures.Methods:A total of 5186 female workers in a petrochemical enterprise were selected,including 4456 menstrual abnormalities,5132 reproductive tract infections and 2653 perimenopausal syndrome according to different types of reproductive health problems.Field questionnaire survey was conducted.Demographic characteristics,basic health information,occupational characteristics,exposure to occupational harmful factors and occurrence of reproductive health problems of female workers were analyzed.Using the occurrence of reproductive health problems(menstrual abnormalities,reproductive tract infections,perimenopausal syndrome)among female petrochemical workers as dependent variables,Will age,culture level,personal income,marriage status,maternal relatives too early or too late menopause,body mass index(BMI),sleep time,physical exercise,length of service,classes of work and daily average working hours,working forms,job position,carrying heavy objects,occupational exposure to hazardous factors,such as personal protection as independent variables,Statistically significant independent variables(P<0.2)were selected.Stepwise regression is used to screen the independent variables,and the statistically significant independent variables screened out by single factor Chi-square test are included in the regression analysis,and a nomogram histogram prediction model is established by logistic regression,and c-index is used to verify the differentiation of the model.Then the Bootstrap method was used to verify the risk model internally from the research object and evaluate the calibration degree of the model.Results: The main occupational hazards in the field investigation include organic solvents(toluene,dinitrotoluene,vinyl chloride,aniline,etc.),dust(silicon dust,welding dust,etc.),toxic gases(carbon monoxide,nitrogen dioxide,hydrogen sulfide,chlorine gas,oil and gas mixture,etc.),electromagnetic radiation,noise,etc.The main positions include oil refining and processing production line positions,inspection,quality inspection positions,logistics management positions,etc.Among them,2,378 are front-line workers,998 are technicians,347 are auxiliary workers,451 are managerial personnel and 958 are service personnel.Workers exposed to occupational-disease-inductive factors are mainly front-line workers(inspection,oil refining)and technical personnel(quality inspection,etc.),while management personnel,logistics personnel and service personnel are less exposed.The age range of 5132 female workers was 26.0-60.0 years old,the average age was(43.63±6.42)years old,and the average length of service was(20.93±8.61)years.96.61% of the female workers were han,and 83.81% were married for the first time.College education was the most,accounting for 1752(34.14%);This was followed by 1,518(29.58 percent)with high school or technical secondary school degrees.Master’s degree or above is the least,accounting for 58(1.13%).The average body mass index(BMI)was(22.38±2.75)kg/m2.Among them,1334(25.99%)took breaks,3 609(70.32%)used to wear masks and gloves,and 2974(57.95%)wore earplugs.Among4456 non-menopausal women,1117(25.1%)had abnormal menstruation in the last 6months.The analysis results showed that age(< 30 years)(OR=0.567,95%CI:0.362-0.889),age(30-35 years)(OR=0.578,95%CI: 0.373-0.894),length of service(≤20 years)(OR=0.790,95%CI: 0.644 ~ 0.969)were the protective factors of abnormal menstruation in female workers(P < 0.05).While age(40~45)(OR=2.364,95%CI: 1.557 ~ 3.591),(45 ~ 50 years old)(OR=5.472,95%CI: 3.287 ~ 9.108),occupational exposure to dust(OR=1.252,95%CI: 1.068-1.468),occupational chemical exposure(OR=1.325,95%CI: 1.128-1.555),not wearing earplugs(OR=1.333,95%CI: 1.133-1.567),premature OR late menopause in maternal relatives(OR=1.206,95%CI: 1.040-1.398),sleep duration(< 6h)(OR=1.246,95%CI:1.056-1.470),leave due to health reasons in recent 6 months(OR=1.532,95%CI:1.251-1.875),physical fatigue(OR=1.575,95%CI: 1.297 ~ 1.911),mental fatigue(OR=1.892,95%CI: 1.577 ~ 2.270),spontaneous abortion(OR=1.248,95%CI: 1.075~ 1.448),reproductive tract infection in recent 6 months(OR=1.598,95%CI:1.359-1.879)was a risk factor for abnormal menstruation in female workers(P <0.05).The ROC curve of the abnormal menstrual line model showed that the C-index index was 0.706(95%CI: 0.689-0.724).The rote line model had good differentiation.Meanwhile,the Bootstrap method was used to verify the risk model internally from the research objects,the sample size was 4456,and the number of re-sampling was1000.The results show that the standard curve and the predicted curve fit well,and the absolute error is 0.008.The calibration degree of the model is good.There were 1241 patients(24.18%)with reproductive tract infection in the last 6 months.Regression analysis showed that occupational electromagnetic exposure(OR=1.392,95%CI: 1.178-1.645),drinking habit(OR=1.291,95%CI: 1.130-1.475),premature OR late menopause in maternal relatives(OR=1.368,95%CI: 1.198 ~ 1.564),breast tumor(OR=1.776,95%CI: 1.182-2.669),leave for medical reasons in recent 6 months(OR=1.482,95%CI: 1.239-1.772),previous history of abortion(OR=1.778,95%CI:1.550-2.041),physical fatigue(OR=1.500,95%CI: 1.260-1.786)and mental fatigue(OR=1.959,95%CI: 1.668-2.300)were the risk factors of reproductive tract infection in female workers(P < 0.05).The ROC curve of the reproductive tract infection model showed that the C-index was 0.655(95% CI was 0.637-0.672),indicating that the rosette model had good differentiation.At the same time,the Bootstrap method was used to verify the risk model internally from the research objects.The sample size was 5132 and the number of re-sampling was 1000.The results showed that the standard curve and the predicted curve in the calibration graph fit well,and the absolute error was 0.005,indicating that the calibration degree of the model was good.There were 1306 cases(49.2%)with symptoms of perimenopausal syndrome(Kupperman score ≥7).The specific symptoms reported were fatigue(79.95%),irritability(71.32%)and insomnia(66.79%).The analysis results showed that age group(> 30 years old)(OR=4.707,95%CI: 1.748-12.674),alcohol consumption(OR=1.207,95%CI: 1.011-1.441),history of abortion(OR=1.685,95%CI:1.414-2.008),physical fatigue(OR=2.393,95%CI: 1.889-3.032),mental fatigue(OR=5.760,95%CI: 4.453-7.451),premature OR late menopause in maternal line(OR=1.574,95%CI:1.320~1.877),sleep time(6h)(OR=1.827,95%CI: 1.493-2.235),no physical exercise(OR=1.567,95%CI: 1.302-1.886),leave due to health reasons in recent 6 months(OR=1.598,95%CI: 1.211-2.108),carrying heavy objects(OR=1.575,95%CI: 1.230-2.018),working position(sitting for a long time)(OR=1.960,95%CI:1.234-3.111),working mode(night shift)(OR=2.152,95%CI: 1.108-4.179)and combined exposure to occupational harmful factors(OR=1.343,95%CI: 1.116-1.615)were risk factors for the occurrence of perimenopausal syndrome in female workers(P < 0.05).The ROC curve of the perimenopausal syndrome model showed that the C-index was 0.734(95%CI: 0.715-0.752),indicating that the line-chart model had good differentiation.At the same time,the Bootstrap method was used to verify the risk model internally from the research objects.The sample size was 2653 and the number of re-sampling was 1000.The results showed that the standard curve and the predicted curve in the calibration graph fit well,and the absolute error was 0.01,indicating that the calibration degree of the model was good.Conclusions: Petrochemical women in reproductive tract infections,menstrual abnormalities,perimenopausal syndrome three reproductive health issues is a process of exposing aroused by many factors,and age,BMI,cultural level,basic health and genetic reproductive problems,social habits,psychological factors,such as industry characteristic of occupational factors are the influence factors of nots allow to ignore,Occupational length of service,occupational exposure to harmful substances(chemicals,dust,noise,electromagnetic),work form,body position,etc.Based on a large sample,this study screened out risk factors and established a risk line graph model for the reproductive health of female petrochemical workers.These lines are helpful to improve the evaluation efficiency of occupational reproductive health risk of female petrochemical workers.It has certain value and practical significance in the monitoring tool of female reproductive health in this industry. |
