Objective: The purpose of this study was to evaluate the outcome of vestibular function in patients with anterior circulation infarction(ACI)complicated with dizziness.The main goals are:(1)the possible site of aci-induced dizziness;(2)Characteristics of vestibular function in ACI patients with dizziness.Methods:A total of 62 patients admitted to the neurology department of Qinghai University Affiliated Hospital from October 2020 to October 2021 and confirmed with ACI by 3.0T head MRI plain scan were collected.According to the assessment criteria for dizziness 2008,the patients were divided into ACI with dizziness group(experimental group)and ACI without dizziness group(control group),with 31 patients in each group.After the collection of general clinical data,all subjects underwent DHI scoring and vestibular function examination.SPSS 25.0 software was used to calculate the difference of vestibular function examination results of ACI infarction site between the experimental group and the control group and the characteristics of ACI infarction site within the experimental group,and finally the experimental conclusion was drawn.Results:1.There was statistically significant difference in DHI score between experimental group and control group(P<0.01);there was no statistical difference in general data(P>0.05),indicating that the baseline data of the two groups were comparable;2.Through the statistical analysis of the lesion sites of ACI in the experimental group and the control group,it was found that there were statistically significant differences in the infarction of the frontal lobe,parietal lobe and insula between the two groups(P<0.05),but there was no statistical significance in the incidence of dizziness in the region related to the center of semicovale in the cingulate cortex of temporal lobe(P>0.05);3.Comparison of video nystagmus and vestibular function between the experimental group and the control group showed that video nystagmus(ST,GT,SPT)showed no significant difference in the discrimination between the two groups(P>0.05),while SN,OPK,position test(Dix-Hallpike test,Supine Roll test)and hot-and-cold air perfusion test had statistical significance for the differentiation between the two groups(P<0.05).Conclusions:1.Patients with ACI(frontal lobe,parietal lobe and insula lobe)are more likely to have dizziness symptoms,and some patients have isolated dizziness as clinical manifestations,which can also be caused by ACI;2.Vestibular function examination results of ACI patients with dizziness have certain diagnostic value,especially abnormal results of SN,OPK,position test(Dix-Hallpike test,Supine Roll test)and hot-and-cold air perfusion test;3.The ACI(frontal,parietal,and insula)region may be part of the vestibular cortex. |
