Objective: Detect the levels of enterotoxigenic Bacteroides fragilis(ETBF)and Fusobacterium nucleatum(Fn)in the feces of patients with colonic polyps in qinghai area,and then detect the levels of ETBF and Fn in the feces of patients with colonic polyps after endoscopic resection and probiotic treatment for 3 months,and analyze their differences in different pathological states,To explore the relationship between fecal ETBF and Fn and the occurrence and development of colonic polyps.Methods: 1.Patients with colonic polyps(n = 50),colon cancer(n = 30)and physical examination(n = 30)who were clearly diagnosed and met the inclusion criteria in the Affiliated Hospital of Qinghai University from November 2020 to June 2021 were selected as the research objects.2.Fecal samples,general information and biochemical indexes of all subjects were collected,and the levels of fecal ETBF and Fn were detected by real-time fluorescence quantitative PCR.3.Select the patients who underwent endoscopic resection in the colonic polyp group and give them probiotics(Bifidobacterium quadruple viable tablet 1.5g/time,3times/day,starting from the second day after operation).After 3 months,collect the fecal samples of these patients and detect the levels of fecal ETBF and Fn.4.Draw a conclusion after statistical analysis of the data.Results:1.The levels of fecal ETBF and Fn in colonic polyp group were higher than those in healthy group(P < 0.05),and lower than those in colon cancer group(P < 0.05).2.The levels of fecal ETBF and Fn in adenomatous polyps group were higher than those in non-adenomatous polyps group(P < 0.05).There was no statistical difference between the levels of fecal ETBF and Fn and the size,number,location,Paris parting of colonic polyps,differentiation degree,distant metastasis,lymph node metastasis and Pathological stage of colon cancer(P > 0.05).3.After endoscopic resection of colonic polyps and probiotic treatment,the levels of fecal ETBF and Fn were lower than those before treatment(P < 0.05).4.The area under the ROC curve(AUC)of fecal ETBF in the diagnosis of colonic polyps was 0.781,the sensitivity was 76%,the specificity was 70%,the best critical value was31734.41 copies /mg,the AUC in the diagnosis of colon cancer was 0.744,the sensitivity was 63%,the specificity was 79%,and the best critical value was53400.50 copies/mg;The AUC of fecal Fn for the diagnosis of colonic polyps was0.817,the sensitivity was 78%,the specificity was 73%,and the best critical value was 23330.50 copies/mg.The AUC for the diagnosis of colon cancer was 0.744,the sensitivity was 63%,the specificity was 79%,and the best critical value was53400.50 copies/mg;When fecal Fn was combined with ETBF,the AUC in the diagnosis of colonic polyps increased to 0.860,with a sensitivity of 82% and a specificity of 80%.When combined with ETBF,the AUC in the diagnosis of colon cancer increased to 0.898,with a sensitivity of 87% and a specificity of 84%.Conclusion: 1.Fecal ETBF and Fn are closely related to the occurrence and development of colonic polyps.2.The levels of ETBF and Fn in feces decreased after endoscopic resection and probiotics treatment in patients with colon polyps,so it has a certain regulatory effect on the levels of intestinal ETBF and Fn.3.The critical values of fecal Fn for the diagnosis of colonic polyps and colon cancer were23330.50 copies/mg and 34411.50 copies/mg respectively,and the critical values of fecal ETBF for the diagnosis of colonic polyps and colon cancer were 31734.41copies/mg and 53400.50 copies/mg respectively.Combined detection of fecal Fn and ETBF levels can significantly improve the detection rate of colonic polyps and colon cancer.4.Detecting the levels of fecal Fn and ETBF can be used as an important auxiliary tool for early screening and diagnosis of colonic polyps.
