Objective The hemodynamic indexes of patients with varicose lower extremity saphenous vein after high ligation and stripping of great saphenous vein were compared between the two pathological types of closed shunt and open deviated shunt,to investigate the effect of high ligation and stripping of great saphenous vein on the two pathological types,for different pathological type choose individualized provide basis for varicose veins surgery.Methods Based on set inclusion criteria and exclusion criteria,in December 2020 to June 2021 were collected during the autonomous region vascular surgery great saphenous varicose veins of lower limb,93 cases of 93 patients with limb,according to the theory of lower extremity varicose veins hemodynamics pathological classification using the double function of color doppler ultrasound instrument to hemodynamic evaluation of limb,The affected limbs were divided into closed shunt group and open bypass shunt group.In the closed shunt group(experimental group),46 limbs were affected,and in the open bypass shunt group(control group),47 limbs were affected.Venous blood flow indexes of lower extremities were measured before and 3 months after surgery: Venous flow index(VAFI);Recirculation index(RCI);Postural diameter change(PDC);Total diameter of femoral common vein(FCVD),Venous filling index(VFI),and SPSS21.0 statistical software were used to analyze the final data.P < 0.05 indicated that there was a significant difference between the experimental group and the control group,and the final results were obtained and discussed.Results Preoperative study group and control group VAFI [(1.58 ± 0.42)and(1.23 ±0.15)],RCI [(4.99 ± 1.34)and(2.23 ± 0.04)],common femoral vein diameter [(12.37± 0.55)and(10.17 ± 0.25)mm],PDC [(17.16 ± 6.96)and(19.98 ± 6.14)%],VFI[(4.89 ± 1.34)and(3.79 ± 1.17)] comparison,the differences were statistically significant(all P < 0.05).Preoperative study group and postoperative study group VAFI [(1.58 ± 0.42)and(1.05 ± 0.21)],RCI [(4.99 ± 1.34)and(2.09 ± 0.38)],common femoral vein diameter [(12.37 ± 0.55)and(10.96 ± 0.78)mm],PDC [(17.16± 6.96)and(21.49 ± 6.55)%],VFI [(4.89 ± 1.34)and(3.15 ± 0.56)]comparison,the differences were statistically significant(all P < 0.05);Preoperative control group and postoperative control group VAFI [(1.23 ± 0.15)and(1.13 ± 0.18)],RCI [(2.23 ± 0.04)and(1.59 ± 0.48)],common femoral vein diameter [(10.17 ± 0.25)and(9.95 ± 0.89)mm],PDC [(19.98 ± 6.14)and(23.12 ± 4.47)%],VFI [(3.79 ± 1.17)and(3.08 ±0.64)]comparison,the differences were statistically significant(all P < 0.05);The change rate of hemodynamic value of affected limb before and after operation in study group and control group VAFI [(0.29 ± 0.21)and(0.05 ± 0.26)%],RCI [(0.48 ±0.26)and(0.19 ± 0.38)%],PDC [(0.55 ± 1.05)and(0.19 ± 0.40)%],VFI [(0.32 ± 0.18)and(0.09 ± 0.37)%]comparison,the differences were statistically significant(all P <0.05).Conclusion 1.High ligation and stripping of great saphenous vein has therapeutic effect on the two hemodynamic types of lower extremity great saphenous vein varices:closed shunt type and open deviated shunt type.2.High ligation and stripping of great saphenous vein is more effective in the treatment of varicose great saphenous vein of closed shunt type.3.Minimally invasive surgical treatment may be more suitable for patients with open deviated branching great saphenous varicose vein with normal saphenous vein valve function,such as hemodynamic correction widely used in recent years.
