Objective:To observe the clinical results after immediate implant surgery in different implant beds for mandibular first molars.METHODS: Patients who needed implantation for unpreserved or missing mandibular first molars between September 2020 and December 2021 in the Department of Oral Implantology of our hospital were divided into immediate implantation groups A,B,C and postponed implantation group.19 implant beds were in group A for teeth that could not be preserved due to periodontitis and needed immediate implantation;17 implant beds were in group B for teeth that could not be preserved due to periapical periodontitis and needed immediate implantation;20implant beds were in group C for teeth that could not be preserved due to residual roots and crowns.There were 25 cases with mandibular first molars missing for at least three months after extraction.The following indices were measured after immediate and delayed implantation: initial and secondary stability of the implant,level of bone resorption at the implant margin,implant probing depth,inter dental papilla index and patient satisfaction,and the differences in clinical outcomes after implantation were evaluated.Results:1.Results of implant stability measurement:There was no statistical significance in initial and secondary implant stability between immediate implant group A,B and C and delayed implant group(P>0.05).2.Measurement results of bone absorption level at implant edge: At the 3rd and 6th month after restoration,there was no statistical significance in the mutual analysis of bone absorption level at implant edge between immediate implantation group A,B and C and delayed implantation group(P>0.05).3.Statistical results of implant probing depth: At the 3rd and 6th month after repair,the comparison between immediate implantation group A and B、C and delayed implantation group was statistically significant(P<0.05),while the comparison between implant probing depth in groups B and C and delayed implantation group was not statistically significant(P>0.05).4.At the 3rd and 6th month after restoration,the PIS scores of implants in groups A,B and C were statistically significant with the delayed implantation group(P<0.05).There was no statistical significance in the comparison of PIS scores between groups A,B and C(P>0.05).5.Patient satisfaction survey results: at the 3rd and 6th month after restoration,there was statistical significance in the satisfaction of implant patients in groups A,B and C compared with the delayed implant group(P<0.05),patient satisfaction in groups A,B and C was better than that in the delayed implant group,and there was no statistical significance in the comparison of patient satisfaction between implants in groups A,B and C(P>0.05).Conclusion:1.The immediate implantation of mandibular first molars with periodontitis,chronic periapical inflammation and crown remnants was feasible,and the immediate implantation was better than delayed implantation in terms of gingival papilla index and patient satisfaction score.2.The differences in the level of bone resorption at the margin of the implant,the gingival papilla index and the subjective satisfaction of the patients were not statistically significant,and it can be concluded that the clinical results after immediate implantation in different implant beds of mandibular first molars were similar.3.The probing depth of periodontitis patients after immediate implantation was higher than that of chronic periapical periodontitis,residual root and crown and delayed implant group.Therefore,immediate implantation chosen for patients with periodontitis should be with due consideration to the implantation conditions and attention to postoperative maintenance.
