Objective:The Liverpool Adverse Events Profile is a 23-item self-examination questionnaire designed to screen for adverse drug reactions in epilepsy.The purpose of this study was to study the evaluation value of The Chinese version of Laep in patients with epilepsy complicated with depressive disorder.Methods:Clinical and imaging data were collected from epilepsy patients who visited the neurology department of Qingdao University Affiliated Hospital from July 2021 to April 2022.All patients did not receive antidepressant treatment and regularly took antiepileptic drugs for more than 1 month.(1)Record the general information and medication of patients,such as age,gender,onset age,medication time,medication type,seizure control,seizure type,etc.(2)Routine blood tests,liver function and renal function tests were performed for each patient.(3)Each patient completed craniocerebral magnetic resonance.(4)Each patient completed The Chinese version of Laep and The Neurological Disorders Depression Inventory for Epilepsy(NDDI-E)at The same time,and recorded The specific scores of The two scales.Analysis was conducted based on the above data:(1)to measure the reliability and validity of the Chinese version of Laep.(2)According to the total score value of The Chinese version of NDDI-E,patients were divided into depression group and control group.The total score value greater than 12 was divided into depression group,and the total score value less than or equal to 12 was divided into control group.Age,course of disease,medication,test results and other indicators were analyzed by univariate and multivariate Logistic analysis to find out the independent influencing factors of depressive disorder in epilepsy patients.Correlation analysis was conducted to further evaluate the correlation between the specific score of each item,the total score,and the number of items greater than 2 points in Chinese version of Laep and depressive disorder in epilepsy patients.(3)Receicer Operating characteristic(ROC)curve was used to analyze the sensitivity,specificity,positive predictive value and negative predictive value of the total score of the Chinese version of Laep and the number of items greater than 2 points for the diagnosis of depressive disorder,and select the cut-off score.Results:Of 344 patients,306 were enrolled.(1)The Cronbach’s α value of Laep Chinese scale was 0.898,indicating good content consistency reliability;For 20 of the 23 items,the correlation coefficient between two measurements was greater than 0.6,and the total correlation coefficient between two measurements was 0.835(P<0.05),indicating good retest reliability;The KMO value of Laep Chinese scale score was 0.889,greater than 0.5,and the Bartlett sphericity test X2 value was 2552(P<0.05)suitable for factor analysis,21 out of 23 items in factor analysis had high load value on one factor(>0.4),suggesting that the scale has good structural validity;Spearman correlation analysis showed that the correlation coefficient between "others" and the total score was only 0.148,and the correlation coefficient between the other 22 items and the total score was 0.240-0.551.The P values of 23 items were all less than 0.05,showing good content validity.(2)306 patients were divided into depression group(71)and control group(235).There was no significant difference in age,education level,duration of epilepsy,duration of medication,type of epilepsy,brain MRI abnormality,duration of medication,co-chronic disease,red blood cell,white blood cell,platelet,albumin,total bilirubin,alanine aminotransferase,aspartate aminotransferase,urea nitrogen and creatinine between the two groups.There were statistically significant differences in gender,age of onset,degree of seizure control,type and quantity of medication,total score of Laep Chinese scale,and number of Laep Chinese scale greater than 2 items(P<0.05).The above 24 indexes were included in univariate regression analysis.Four indexes,including type and quantity of medication,degree of control,total score of Chinese Version of Laep,and number of more than 2 items of Chinese version of Laep were the influencing factors of depressive disorder in epilepsy patients(P<0.05).Multivariate regression analysis suggested that a variety of antiepileptic drugs,total score of the Chinese version of Laep,and the number of items of the Chinese version of Laep greater than 2 were independent influencing factors of depressive disorder in epilepsy patients(P<0.05).(3)The total score of Laep Chinese scale was included in ROC analysis,indicating that the area under the curve was 0.877(95%CI=0.819-0.936,P<0.05),when the truncation score≧41,the sensitivity and specificity of the total score were 0.860 and 0.859;when the truncation score of the Chinese version of Laep scale was greater than 2 items and the truncation score was≧5,the sensitivity and specificity were 0.838 and 0.944.The results of univariate Logistic regression analysis showed that the scores of 23 items in The Chinese version of Laep scale had no predictive value for depressive disorder(P<0.05).Conclusions:(1)The Chinese version of Laep is simple and effective,and the scale itself has good reliability and validity.(2)The total score of the scale and the number of items greater than 2 points can be used to evaluate depressive disorders in epilepsy patients.(3)Taking a variety of antiepileptic drugs was an independent risk factor for depressive disorder in patients with epilepsy.
