Objective:Focused ultrasound ablation surgery(FUAS)plays an important role in the nonsurgical treatment of hysteromyoma, but it takes a long time to improve the clinical symptoms and absorb the necrotic tissue after FUAS treatment, which affects the prognosis and quality of life of patients.Traditional Chinese medicine Zhengjia recipe can improve the clinical symptoms of patients with hysteromyoma, but the effect and mechanism of its combination with FUAS in the treatment of hysteromyoma have not been studied. This study will explore the effect of FUAS combined with traditional Chinese medicine Zhengjia Recipe on the postoperative clinical symptoms, fibroid volume and TCM symptom score of patients with hysteromyoma. Methods:The clinical data of patients with uterine fibroids treated by focused ultrasound ablation in Laizhou Maternal and Child Health Hospital from June 2019 to June 2020 were retrospectively analyzed.A total of 77 patients were selected, before the operation, the dialectical evidence through traditional Chinese medicine belongs to the blood stasis type, and they were divided into study groups according to whether they were treated with Zhengjia prescription after surgery.(n=38)and control group(n=39).The patients in the control group were treated with FUAS alone, while the patients in the research group were treated with Zhengjia prescription,a traditional Chinese medicine,on the first day after FUAS to avoid menstrual periods for 3 months. The improvement of clinical symptoms and the volume change of uterine fibroids in the two groups were observed at 1,3,6,and 1year after treatment. The clinical symptoms included lower abdominal mass, menstrual flow, frequent urination and fatigue, etc.to evaluate. This study was approved by the Medical Ethics Committee of Laizhou Maternal and Child Health Hospital, and the treatment was informed by the patients or their families, and the informed consent was signed.Results:(1)There was no significant difference in clinical data and other baseline data including age, fibroid number, fibroid volume, fibroid location, fibroid type, etc.between the two groups(P>0.05),and they were comparable.(2)There were no significant differences in the operation time, irradiation time, treatment intensity and ablation rate between the two groups of patients during focused ultrasound ablation(P>0.05).During the treatment, there was no significant difference in the radiating pain, sacrococcygeal pain, skin burning, pain in the treatment area, groin pain and other symptoms and adverse reactions between the two groups, and the symptoms were mild, and the symptoms were relieved without special intervention(P>0.05).The two groups were comparable.(3)There was no significant difference in liver function(ALT,AST)between the two groups before treatment and 3 months after treatment, indicating that the traditional Chinese medicine Zhengjia recipe has less effect on liver function and higher safety.(4)The clinical symptoms at 1 month,3 months,6 months and 1 year after treatment in the two groups:lower abdominal mass, menstrual volume, frequent urination, fatigue, etc.were improved compared with before, and the clinical symptoms of the research group improved significantly better than the control group(P<0.05),the difference was statistically significant.(5)There was no significant difference in the myoma reduction rate between the two groups at 1 month after treatment(P>0.05),but the myoma reduction rate at 3months,6 months and 1 year was better than that of the control group(P<0.001).(6)There was no statistical difference in symptom scores between the two groups before treatment(P<0.05).After treatment, the symptom scores of the observation group were lower than those of the control group(P<0.05),and the clinical efficacy was better than that of the control group(P<0.05).(7)Among the patients in the study group who took traditional Chinese medicines, one patient had mild nausea after taking traditional Chinese medicines, and the symptoms improved after adjusting the medication time. One patient was slightly upset and felt itching and discomfort at the beginning of taking the medicine. No special treatment was given, and then the symptoms disappeared on their own. Conclusion:The treatment of uterine leiomyoma by FUAS combined with traditional Chinese medicine Zhengjia recipe will help to improve the clinical symptoms, reduce the volume of postoperative leiomyoma, reduce the score of postoperative TCM symptoms, and significantly improve the quality of life of patients. This treatment scheme has certain safety and feasibility, and can be popularized in clinical practice.
