ObjectiveTo observe the effects in patients with coronary slow flow after regular application of Nicorandil,Shexiang Baoxin Pill and their combination therapy for 3 months and using three-dimensional speckle tracking technology in patients with coronary slow flow condition.MethodsFrom October 2019 to October 2021,120 patients with coronary slow flow confirmed by coronary angiography in the Department of Cardiology,Affiliated Hospital of Qingdao University were prospectively investigated,and they were randomly divided into three groups(number table method).40 cases of nicorandil group(nicorandil 5 mg three times a day),40 cases of shexiangbaoxin pill group(shexiangbaoxin pill 45 mg three times a day),40 cases of combined group(nicorandil 5 mg 3 times a day;shexiang baoxin pill45 mg 3 times a day).Observation of patient indicators before and 3 months after treatment: routine indicators: triglyceride(TG),total cholesterol(TC),low density lipoprotein(LDL),high density lipoprotein(HDL)and lipoprotein a [Lp(a)];Threedimensional speckle index: left ventricular global longitudinal strain(GLS),left ventricular global radial strain(GRS),left ventricular global circumferential strain(GCS),left ventricular global area strain(GAS);echocardiography index: left ventricular enddiastolic diameter(LVDd),left ventricular end-systolic diameter(LVDs),ejection fraction(EF value).Symptoms: chest pain,chest tightness,incidence of arrhythmia.Finally,the clinical data of the patients were collected for analysis and comparison.Results1.Comparison of each group after taking the medicine and before taking the medicine1.1 Biochemical indexes of nicorandil group: TG and LDL decreased compared with before taking drug,HDL increased compared with before taking drug;three-dimensional speckle tracking index: GLS value decreased compared with before taking drug,and the above indexes were all statistically significant(P<0.05);1.2 Biochemical indexes of Shexiang Baoxin Pill group: TG,LDL,Lp(a)decreased compared with those before taking medicine;three-dimensional speckle tracking indexes:GLS and GRS values decreased compared with those before taking medicine,and the above indexes were all statistically significant(P<0.05);1.3 Biochemical indexes in the combination group: TG,TC,LDL,Lp(a)decreased compared with those before medication,and HDL increased compared with those before medication;two-dimensional echocardiography indexes: EF values increased compared with before,three-dimensional speckle tracking indexes: GLS and GAS values compared with those before medication The above indexes were all statistically significant(P <0.05).2.The three groups before and after taking the drug were statistically significant for comparison between groups2.1 Triglyceride: The difference between the combination group and the Shexiang Baoxin Pill group before and after taking the medicine was statistically significant(P<0.05),and there was no statistical difference between the combination group and the nicorandil group,and the nicorandil and Shexiang Baoxin Pill group.academic differences(P>0.05).2.2 Low-density lipoprotein: the difference between the combination group and the other two groups before and after taking the drug was statistically significant(P<0.05),while the difference between the nicorandil group and the Shexiang Baoxin Pill group was not statistically significant(P>0.05);2.3 Overall longitudinal strain of the left ventricle: Compared with the other two groups,the overall longitudinal strain of the left ventricle was significantly different before and after taking the medicine(P < 0.05),while the difference between the nicorandil group and the Shexiang Baoxin Pill group was significantly different There was no statistical significance(P>0.05).3.Comparison between the three groups after taking the drug3.1 The low density lipoprotein and the overall longitudinal strain of the left ventricle were statistically different among the three groups(P<0.05);3.2 Pairwise comparison of low-density lipoproteins in the three groups after taking the medicine shows that there is no statistical difference between the combination group and the nicorandil group,but there is a statistical difference between the combination group and the Shexiang Baoxin Pill group(P<0.05);3.3 Pairwise comparison of the overall longitudinal strain of the left ventricle in the three groups after taking the medication,it can be seen that the overall longitudinal strain of the left ventricle in the combined medication group has a more significant change than before medication,and there is a statistical difference compared with the other two monotherapy groups(P<0.05).4.The effective rate of single drug treatment in Nicorandil group and Shexiang Baoxin Pill group was about 80%,while the effective rate of combined drug treatment was97.5%.(P<0.05)ConclusionThe research results show that the three groups of drugs can reduce TG,TC,LDL,Lp(a)to varying degrees,increase HDL,and improve the overall longitudinal strain of the left ventricle,the index of myocardial contractility,but the combination group reduces lowdensity lipoprotein and triglyceride.It was more obvious that the overall longitudinal strain of the left ventricle was better improved,the myocardial contractility was enhanced,and the remission rate of clinical symptoms in the combined drug treatment group was higher and the effect was more significant.
