ObjectiveTo investigate the current situation of hope level of lung cancer targeted therapy patients and analyze its influencing factors, explore the hope experience of lung cancer targeted therapy patients, and initially construct a scientific and practical hope intervention plan applicable to lung cancer targeted therapy patients,so as to provide reference and basis for hope intervention of lung cancer targeted therapy patients.Methods1.A study on the level of hope and influencing factors of patients with targeted therapy for lung cancer. In this part,a cross-sectional study design was adopted, and 103 lung cancer patients who were treated with oral targeted drugs from July 2021 to November 2021 in the oncology department of a tertiary care general hospital in Weifang City were selected by convenience sampling method, and a two-dimensional code questionnaire was generated by the Questionnaire Star platform, and patients scanned the code to fill in the questionnaire, which included a general information questionnaire,a hope level scale,a medical coping style The questionnaires included general information questionnaire, hope level scale, medical coping style questionnaire, and social support scale. The data collected by Questionnaire Star were imported into SPSS25.0 database, and descriptive analysis,t-test,one-way ANOVA,Pearson correlation analysis, and multiple stepwise regression analysis were used to analyze the data.2.Hope experience study of patients with targeted therapy for lung cancer. This part adopts qualitative research, purposive sampling method to extract 13 patients of targeted lung cancer treatment as research subjects, using semi-structured interview method to collect patients’ real hope experience, applying Colaizzi 7-step analysis method to analyze, summarize and refine the themes.3.Study on the construction of a hope intervention program for patients with targeted lung cancer therapy. On the basis of literature review, combining the quantitative and qualitative research results, the preliminary draft of the hope intervention program for patients with targeted lung cancer therapy was constructed with the guidance of hope theory, and two rounds of correspondence consultation were conducted with 18 experts using the Delphi method, and the reliability of the correspondence results was tested by using the positive coefficient, authority coefficient, and the degree of concentration and coordination of experts’ opinions to determine the final hope intervention program for patients with targeted lung cancer therapy. The results were tested for reliability using the Delphi method.Results1.Questionnaire results:(1)103 patients with lung cancer targeted therapy had a hope level score of(39.22±5.32),which was generally at a high level,of which 79.6%were at a high level and 20.4% were at a medium level. The scores of each dimension in descending order were: positive attitude toward reality and future(13.37±2.15),taking positive actions(13.06±2.19),and maintaining close relationships with others(12.80±2.56).(2)The values of the correlation between the level of hope and the 5 items of confrontation,avoidance,submission,extra-family support, and family support showed significant(r values of 0.457,-0.370,-0.461,0.509,and 0.506,respectively,P<0.05).(3)Using the level of hope as the dependent variable, variables with significant differences in the one-way ANOVA,including personality disposition, work status, monthly per capita household income and medical coping style(confrontation, avoidance, submission)and comprehending social support(extra-familial support, family support)as independent variables, were subjected to multiple stepwise regression analysis, and the four influencing factors that finally entered the regression equation model were household per capita monthly income,extra-familial support, confrontation, and submission. The explanatory power of the independent variables on the dependent variable was 45.8%.Extra-family support, per capita monthly household income, and confrontation had significant positive effects on the level of hope(regression coefficients of 0.128,1.606,and 1.256,respectively,P<0.05),and submission had significant negative effects on the level of hope(regression coefficient of-1.581,P<0.05).2.Results of semi-structured interviews: by analyzing the data of 13 interview subjects, five themes were extracted: The dynamic nature of hope, the source of hope, the power of hope, the factors that hinder hope, the understanding of hope.3.The results of constructing the hope intervention program: the two rounds of Delphi correspondence established the hope intervention program for patients with targeted lung cancer, which consisted of 6 intervention modules,18 intervention topics,36 interventions, and determined the composition, division of labor, training, and intervention format of the intervention team. The positive coefficients of the experts in the two rounds of consultation were 90% and 100%,and the authority coefficients of the experts were greater than 0.80.The experts’ evaluation of the rationality of the intervention form, team composition, division of labor, and training form of the program in the two rounds of consultation were greater than 90%,and the Kendall coordination coefficients were 0.628 and 0.844(P < 0.01),respectively,which showed a high degree of coordination among the experts and reliable results of the consultation.Conclusion1.Patients with lung cancer targeted therapy have a high level of hope.Patients’ hope level is significantly correlated with medical coping style and social support, and hope level is influenced by various factors. In clinical practice,we should focus on patients with introverted personality, low per capita monthly family income, farming or no work, and improve their hope level by improving their medical coping style and increasing social support.2.Hope shows obvious dynamic changes in the course of lung cancer targeted therapy patients, and hope is a process of continuous internalization with disease development.Hope is the motivation for patients to live actively and treat the disease, which can help them avoid stress and control their emotions.Health care professionals should pay attention to the process of change of their hope and strengthen the intervention of hope at important points, which can strengthen patients’ hope experience in terms of strengthening internal spiritual beliefs, increasing family support, increasing professional support and encouraging them to live in the present moment.3.The hope intervention program for patients with targeted lung cancer therapy constructed in this study is scientific, targeted and reliable, which can provide reference and basis for hope intervention for patients with targeted lung cancer therapy... |
