ObjectivesAnalyze the related risk factors of white matter osteoporosis,and explore the influence of white matter osteoporosis on cognition and emotion,so as to provide reference for the prevention of white matter osteoporosis and improve the awareness and attention of clinicians.MethodsA continuous retrieval was performed for the information of patients hospitalized in the Department of Neurology 1 of the Second Affiliated Hospital of Guangzhou Medical University from 2021.01.01 to 2021.12.31,who had received cranial magnetic resonance imaging(MRI).According to the head MRI report,patients can be divided into two groups.Head MRI showed white matter lesion(WML)in the leukoaraiosis group,and head MRI showed normal in the control group.The information about age,sex,education level,history of hypertension,coronary heart disease,type 2 diabetes,smoking and alcohol consumption,SAA,APTT,PT,INR,ApoAI,ApoB and their ratio of ApoAI and ApoB,TG,HDL-C,LDL-C,TC,glycated hemoglobin,creatinine,HCY,uric acid,fasting blood glucose,erythrocyte sedimentation rate,HAMA score,HAMD score,MMSE score of these two groups were collected.Appropriate statistical methods were used to find out the related risk factors of white matter osteoporosis,and explore whether they had an impact on cognition and mood.ResultsA total of 120 patients meeting the requirements were collected.The experimental group accounts for 68 cases,including 24 males and 44 females,with an average age of 72.18±8.46 years;The control group consists of 52 cases,including 18 males and 34 females,with an average age of 50.46±8.66 years.1)Univariate of analysis between the experimental group and the control group shows that age,hypertension,type 2 diabetes,coronary heart disease,glycosylated hemoglobin,LDL-C,creatinine,HCY and education level have statistical significance(P<0.05).Gender,smoking,alcohol consumption,fasting blood glucose,SAA,APTT,PT,INR,TC,TG,HDL-C,ApoAI,ApoB and their ratio of ApoAI and ApoB,uric acid and ESR have no statistical significance(P>0.05).2)Multifactor binary logistic regression analysis shows that age,hypertension,type 2 diabetes,coronary heart disease and HCY are independent risk factors of white matter osteoporosis.3)MMSE score below 27 points is defined as cognitive impairment.the experimental group has the number of cognitive impairment at 39 people,not the number of cognitive impairment at 29.the control group has the number of cognitive impairment at 9 people,not the number of cognitive impairment at 43.the experimental group has a higher incidence of cognitive impairment than the control group,with statistical significance(P<0.05).the aspects of time orientation,immediate memory,computing power,delayed memory and language of patients are mainly affected,with statistical significance(P<0.05).but there are no statistical significance in spatial orientation and visual space of other aspects of MMSE(P>0.05).4)Leukoaraiosis group can be divided into paraventricular white matter degeneration and paraventricular combined subcortical white matter degeneration.Compared with paraventricular combined subcortical white matter degeneration,paraventricular white matter degeneration has statistical significance in attention and calculation(P<0.05),but not in time orientation,spatial orientation,immediate memory,delayed memory,language and visual space(P>0.05).5)The HAMA and HAMD total score of the experimental group are higher than those of the control group,with statistical significance(P<0.05).Conclusion1.Age,hypertension,type 2 diabetes,coronary heart disease and HCY are related with leukoaraiosis incidence which are independent risk factors for leukoaraiosis.2.Compared with the normal group,patients with white matter osteoporosis have a higher incidence of cognition impairment,which mainly affect the aspects of time orientation,immediate memory,attention and computation,delayed memory and language of patients.3.Compared with paraventricular combined subcortical white matter degeneration,paraventricular white matter degeneration mainly affects patients’attention and computational power.4.Patients with white matter osteoporosis are more likely to experience adverse emotion than the normal group,including anxiety and depression. |
