| Objective Compare the clinical effect of biological lengthened stem artificial femoral head replacement and PFNA in the treatment of elderly patients with unstable intertrochanteric fracture.Method From October 2015 to October 2019,Clinical date of seventy-six elder osteoporotic patients diagnosed intertrochanteric fracture..According to the difference of surgical treatment options,the patients were divided into observation group(38 cases)and control group(38 cases).The treatment scheme of observation group was biological lengthened stem artificial femoral head replacement The control group was treated with Proximal Femoral Nail Antirotation.The operation indexes and clinical indexes were compared between the two groups before and after treatment.The main evaluation criteria included the operation time,blood loss,incision length,the time of first weight-bearing after operation,and the incidence of postoperative complications.The hip joint function of the two groups was followed up after 12 months of treatment Harris and VAS were compared postoperative complications and the MOS 36-item Short Form Health Survey(SF-36)score at different postoperative time points were compared between the two groups.Result By comparing the two groups of patients with surgical indicators and clinical indicators,it can be found that the observation group in the first postoperative weight-bearing time,hospitalization time was significantly shorter than the control group(P < 0.05);and after follow-up,the VAS score of the observation group was significantly lower than that of the control group(P < 0.05);and the two groups of patients completed the treatment after 3 months,6 months the Harris score of the observation group was significantly higher than that of the control group(P < 0.05).And after the comparison of intraoperative treatment indicators,it was found that the average operation time and incision of the observation group were significantly shorter than those of the observation group.Compared the incidence of postoperative complications between the observation group and the control group(P > 0.05),The scores of each dimension of SF-36 scale in the treatment group were higher than those in the control group 3m after operation(P<0.05),but there was no significant difference in each dimension score of SF-36 scale between the two groups 12 m after operation(P>0.05).Conclusion with PFNA,the long-stem artificial femoral head replacement can effectively shorten the off-bed activity time with better recovery of hip function in the early stage and higher quality of life,it is a good choice for patients with severe osteoporotic femoral intertrochanteric fractures.Figure 2 table 7 reference 48[amount]... |
