Background:Dentition defect is a common oral disease among the elderly.With the aging population gradually becoming in the irresistible trend of development,the number of patients diagnosed as dentition defect is increasing.According to the latest National Oral health Epidemiological Survey Report,81.7% of the elderly from 65 to74 live with incomplete dentition.There are several restoration method,including removable partial dentures,fixed-removable combined dentures and implant dentures.Though they are considered to be aesthetic,comfortable and well-functioned,implant dentures and fixed-removable combined dentures still have to strictly follow the indications.So far,removable partial dentures form the primary sector of restoration on account of its extremely wide indications,minimal invasiveness,reversibility,short treatment process and low prices.In recent years,the government invest more and more in constructing public entertainment places for the elderly,so that spare time activities,senior universities and square dances greatly enrich their lives.Living in a more sociable way increases their demands for more aesthetic dentures.Those metal clasps surrounding the buccal and lingual teeth surface are easily exposed when anterior teeth and premolar abutments are designed to place traditional RPD clasps,which seriously destroy the dentures aesthetic and reduces the acceptance degree of traditional clasp RPD.Therefore,it is still of great significance to study and balance the functionality and better aesthetics at the same time through rational restoration design.Objective:To investigate the clinical effects of different design aesthetic clasp in the dentition defect RPD restoration of and the patient’s satisfaction degree of the aesthetic and functional denture after restoration.Materials and methods: This study involved 10 patients diagnosed as dentition defect and received treatment in the Affiliated Stomatological Hospital of Dalian Medical University from June 2020 to November 2021.All patients treated with RPD restoration had reserved anterior teeth and premolars abutments.Among 10 patients aged from 58 to 83 with an average age of 69,including 3 males and 7 females,there were 9 maxillary restoration cases and 5 mandibular cases.It was necessary to ensure that chief complaint and desired expectation of patients have been fully understood,examination results and treatment procedures have been notified,and the final protocol has been made according to patients preference before the restoration treatment started.The aesthetic retention area was determined by modifying the direction of the denture emplacement path or the axial surfaces form of the abutment teeth,then to design the RPD clasp considering both aesthetics and retention,finally to perform the satisfaction investigation into its function and aesthetics.Results: 10 final dentures were completely in place at the time of attempting with every denture base fitted both soft and hard tissue well to providing appropriate retention,and there was no warping or wobbling while they were functioning.2 weeks after the treatment,3 patients felt local mucosa tenderness point existing which could relief after modified.Only 1 patient reported excessive tight contact of posterior teeth on the right side which could improved after modified.All patients were satisfied with the denture function and aesthetics for the anterior tooth clasps were highly concealed.There was hardly any exposure of clasps and no dentures loosening or falling off during use.Conclusion: When using anterior teeth and premolars as abutments for dentition defects RPD restoration,the design of aesthetic clasps considering casts observation and abutments preparation could achieve satisfactory aesthetics and functions in the meanwhile,thereby improving the acceptance of clasp RPD.
