Objective: to observe the effect of fire needle on Nrg1/ErbB2 signal pathway and motor function recovery in rats with sciatic nerve injury.Methods: 54 SD male rats were randomly divided into sham operation group(n=18),model group(n=18)and fire needle group(n=18)by SPSS software.The model of sciatic nerve clamp injury was made in the model group and fire needle group,and only the sciatic nerve was exposed in the sham operation group.In the fire needle group,Huantiao and Weizhong points were treated with fire needle at 24 hours after successful modeling,once every two days,the treatment cycle was 14 days,a total of 7 times.The sham operation group and the model group only performed the same restraint action as the fire needle group.Sciatic nerve function index(Sciatic functional index,SFI),hematoxylin-eosin staining(hematoxylin-eosinstaining,HE),transmission electron microscopy(Transmission electronmicroscope,TEM)and immunohistochemical technique(immunohistochemistry)were used to detect the recovery of motor function,hematoxylin-eosin staining(hematoxylin-eosinstaining,HE),transmission electron microscopy(Transmission electronmicroscope,TEM)and immunohistochemistry(immunohistochemistry)in the three groups after 1 day,7 days and 14 days respectively.IHC)to detect the expression of Nrg1,ErbB2,S100 β and MBP protein in the injured sciatic nerve.Results: 1.after 1 day of treatment,the SFI score of fire needle group and model group was lower than that of sham operation group,and there was no significant difference between the two groups(p > 0.05).After 7 days and 14 days of treatment,the SFI score of fire needle group and model group increased significantly,and the SFI score of fire needle group increased more significantly than that of model group.2.1 day after treatment,the local nerve fibers in the sham operation group were arranged neatly,the thickness and shape were the same,and the axons and myelin sheath were intact,and the local nerve tissue was injured in both the model group and the fire needle group.such as nerve fiber rupture,tissue fragments,axon disintegration,demyelination and so on.After 7 and 14 days of treatment,there was no significant improvement in the model group,but clear repair of injured nerve fibers and proliferation of Schwann cells(schwann,SCs)were observed in the fire needle group.The recovery of local nerve tissue was better than that in the model group.3.After 1 day of treatment,the myelin sheath of the model group and fire needle group was damaged and broken,and the organelles were deformed and swollen,etc.After 7 and 14 days of treatment,the regeneration of myelin sheath was not obvious in the model group,and a large number of new myelin sheath appeared in the fire needle group,and the myelin sheath was thicker.The structure and function of organelles recovered well,and the overall repair was better than that of the model group.4.1 day after treatment,the expression of Nrg1,ErbB2 and S100 β protein in the model group and fire needle group was significantly lower than that in the sham operation group,while the expression of MBP protein was significantly higher than that in the sham operation group(p < 0.01),and the expression of Nrg1,ErbB2,S100 β and MBP protein in the fire needle group was significantly higher than that in the model group after 7 days and 14 days of treatment.Conclusion: 1.Fire needle can promote the recovery of motor function in rats with sciatic nerve injury.2.Fire needle can promote axon repair and myelin regeneration at the injured part of sciatic nerve in rats,and obviously promote the repair of organelles and the proliferation of SCs at the injured site.3.Fire needle can promote the expression of Nrg1,ErbB2,S100 β and MBP protein at the site of sciatic nerve injury in rats.4.Fire needle promotes SCs proliferation and myelin repair by activating Nrg1/ErbB2 signal pathway,which may be one of the related molecular pathways of fire needle promoting repair and regeneration of peripheral nerve after injury.
