| Objective:To study the clinical endoscopic and pathological characteristics of colorectal serrated polyps and summarize the pathogenesis characteristics of colorectal serrated polyps,so as to facilitate clinicians to better understand serrated polyps and promote the diagnosis and treatment of serrated polyps in clinical work.Methods:A retrospective analysis was performed on patients WHO underwent colonoscopy in Shanxi Provincial People’s Hospital from June 2017 to June 2021 and were diagnosed with serrated polyps after pathological section observation by senior pathologists according to the latest classification and diagnosis criteria of WHO.The general information,clinical manifestations,pathological morphology,site and pathological results of the included patients were collected,and the effectiveness and safety of the patients undergoing ESD were statistically analyzed.Results:1.Age and gender: Among 253 patients,118 were male(46.64%)and 135 were female(53.36%),with an average age of(52.26±11.53)years.2.Clinical manifestations: 53 patients diagnosed as SP had abdominal discomfort,accounting for 20.95%;13 patients had stool habit change,accounting for 5.14%;13patients had blood in stool,accounting for 5.14%;2 patients were hospitalized due to anemia and emaciation,accounting for 0.79%;172 patients were examined,accounting for 67.98%.3.Lesion sites: 158 cases(64.45%)were located in the right colon,and 75 cases(37.55%)were located in the left colon.4.Endoscopic features: Lesion size: there were 63 patients with lesions ≤0.5cm,accounting for 24.90%;The lesion size of 103 patients was 0.6-1.0cm,accounting for40.71%;The lesion size of 61 patients was 1.1-2.0cm,accounting for 24.11%;Lesion size >2.0cm in 26 patients,accounting for 10.28%.Pathological morphology: 46 patients were flat and uplifted,accounting for 18.18%;There were 143 patients with broad basal disease,accounting for 56.52%.29 patients had pedicled eminence,accounting for11.46%;35 patients had lateral developmental mass,accounting for 13.83%.5.Pathology: according to WHO classification criteria,30 patients were HP,accounting for 11.86%;138 patients were SSP,accounting for 54.55%;Eighty-five patients(33.60%)were TSA.There were 199 patients without abnormal polyp development,accounting for 78.66%;48 patients had abnormal polyp development,accounting for 18.97%;6 patients had polyps with canceration,accounting for 2.37%.6.ESD treatment: Among the 253 patients included,39 patients underwent ESD,including 17 males and 22 females,with an average age of(60.13±11.53)years,27 patients with lesions located in the left semicolon and 12 in the right semicolon;The mean hospital stay was 8 days,and the mean operation time was 87 minutes.ESD surgery was successfully completed in all 39 patients,with an overall resection rate of100%(39/39)and an average wound length of 4.01 cm.Within 3 days after operation,8cases had fever and 31 cases had no obvious discomfort.Conclusion:Colorectal serrated polyps have high malignant potential and short malignant cycle,and have some characteristics of their own,suggesting that endoscopists should pay more attention to serrated polyps and achieve early detection,diagnosis and treatment according to their own characteristics,so as to reduce the incidence of colorectal cancer. |
