Objective:To detect the expression of plasma oxidized-high density lipoprotein(ox-HDL)in children with Kawasaki disease(KD),and investigate its value in predicting Kawasaki disease complicated with coronary artery lesions. Methods:Collected 100 children with Kawasaki disease in Children’s Hospital of Shanxi from September 2020 to September 2021.Children were divided into two subgroups according to the reports of color Doppler echocardiography of coronary artery: NCAL group(70 cases,non-coronary artery lesion group)and CAL group(30 cases, coronary artery lesion group),collected their serum samples in acute phase and subacute phase. In the same period,100 healthy controls and100 fever controls(fever ≥5 days with respiratory tract infection who were hospitalized for the first time)were enrolled into the study and collected their serum samples at once. The levels of plasma ox-HDL were detected in each group by ELISA.Recorded the laboratory parameters(WBC,NEUT#,HB,PLT,CRP,ESR)of the children with Kawasaki disease.Data analysis(SPSS 25.0 software). Results:In acute phase, the plasma ox-HDL levels in children with KD(both CAL and NCAL)were significantly higher than those in the control groups(P <0.05);Compared with KD group, the level of plasma ox-HDL in coronary artery lesion group(CAL group)increased more significantly(P<0.05).;In the acute stage of children with KD the levels of NEUT#,CRP and ESR in the CAL group were higher than those in the NCAL group(P<0.05).Compared with the NCAL group, the CAL group had no significant differences in HB,PLT,WBC(P>0.05);Pearson correlation analysis found that in acute phase the level of plasma ox-HDL in children with KD were positively correlated with NEUT# and ESR(P<0.05),but no correlation with WBC,HB,PLT,and CRP(P>0.05).Both plasma ox-HDL level and the NEUT# were independent risk factors for CAL in children with KD.The ROC curve showed that Ox-HDL≥46.11(ug/L),the sensitivity of 77% and specificity of 97%,predicting KD complicated with CAL.Compared with the acute phase, the plasma ox-HDL levels in the subacute phase of KD CAL group and NCAL group were decreased, but the difference was not statistically significant(P>0.05).Conclusion:The plasma ox-HDL levels of children with KD children is increased, especially when combined with CAL.Highly levels of plasma ox-HDL may be involved in the process of coronary artery damage in KD,which may be related to the sustained damage. The level of Ox-HDL is expected to be an early predictor of coronary artery lesion in Kawasaki disease. |
