| Objective:This study focuses on the present situation of the allocation of oral health resources in Shanxi Province.Through the investigation and analysis of oral health institutions and oral health human resources in Shanxi Province, the existing pattern and distribution characteristics are discussed. It provides reference for relevant departments to rationally optimize resource allocation, realize efficient utilization of existing oral health resources, and make oral health resources better serve the people’s ever-increasing oral health needs. Methods:The survey was conducted using a self-administered questionnaire, and the basic information of dental practitioners in the registered dental health institutions and institutions in 11 prefecture-level cities in Shanxi Province was collected through a census, and the time point of the surveyed information was December 31,2020.The collected information was collected and entered, then logically screened, screened without error to establish a database, and finally analyzed the data.Results:1.Shanxi Province has a total of 2557 dental health institutions, including 417 public institutions(16.31%)and 2140 non-public institutions(83.69%).The average population served by each oral health care institution is 13,700,and the average service area of each oral health care institution is 61.26 square kilometers.2.The total number of dental practitioners in Shanxi Province is 12525,of which6596 are dentists,5128 are dental nurses and 801 are dental technicians, with a corresponding ratio of 1:0.78:0.12.Dentists account for 6.19% of the total number of clinicians, the population ratio of dentists in the province is 1:5292,the density of dentists is 18.90,and each dentist The average service area per dentist was 23.75 square kilometers.Dentists are classified according to gender, male and female accounted for40.80% and 59.2% respectively;according to the classification of title level, the proportion from high to low is junior title(49.70%),no title(26.51%),intermediate title(17.59%)and senior title(6.20%);according to the classification of education level, the proportion from high to low is College degree holders(55.50%),undergraduate degree holders(31.70%),master’s degree holders and above(7.89%)and less than college degree holders(4.91%).The distribution of both titles and education of dentists showed that public dental health institutions were higher than non-public dental health institutions(P<0.05);urban dental health institutions were higher than rural dental health institutions(P<0.05).3.There was a negative linear relationship between the density of dentists and regional geographic area in Shanxi Province(r=-0.604,P<0.05),no significant correlation between the number of regional population(r=0.176,P>0.05),and no significant correlation between the regional economy(r=0.229,P>0.05).The HRDI of oral health care resources in each prefecture-level city showed that the overall allocation of oral health care resources in Taiyuan,Yangquan,Yuncheng and Jincheng cities was better than that in other prefecture-level cities in Shanxi province. The distribution of oral health care resources in Shanxi province has different degrees of inequity in different dimensions, and the equity is generally characterized by a better distribution by resident population than by economy, and a better distribution by economy than by geographical area.Conclusion:1.Among the oral health institutions in Shanxi Province, the number of non-public institutions far exceeds that of public institutions, but the overall number is still insufficient and the structural distribution is uneven, showing huge regional differences, among which, the distribution of institutions in the southern and central regions of Shanxi Province is better than that in other areas of Shanxi Province. It is suggested to guide the healthy development of non-public oral health institutions.2.There is still a certain gap in the number of oral health human resources in Shanxi Province;the degree of stomatologists is mainly junior college, the professional title is mainly junior, the overall level is low;the proportion of internal health care structure is out of proportion, and the number of oral support staff is seriously insufficient. It is suggested to strengthen the cultivation of oral health talents, improve their quality and service level, and optimize the medical and nursing composition of oral health human resources.3.The allocation of oral health resources in Shanxi Province is unfair from the perspective of geography, population and economy. It is suggested to further improve the rational allocation of oral health resources and form a diversified allocation of health resources to ensure the sustainable development of stomatology in Shanxi Province. |
