Objective:To analyze the clinical value of superb microvascular imaging in quantitative evaluation of blood flow during tibial transverse transport of diabetic foot.Methods:Retrospective analysis was conducted on the clinical data of 18 patients with diabetic foot who were admitted to the Department of Orthopedics,the second Hospital of Shanxi Medical University from November 2019 to November 2021.All the patients were treated with tibial transverse transport to promote wound healing under superb microvascular imaging monitoring.There were 13 males and 5 females,aged(60.79±16.04)years(range: 35 to 90 years).During the treatment,color Doppler ultrasonography was used to observe the changes of arterial wall and lumen of lower limbs of the patients.Then the superb microvascular imaging mode was switched to observe the new blood vessels and calculate the changes of parameters such as vascular index and blood flow,and compare and analyze the superb microvascular imaging results before and after treatment.The WIFi grading was used to evaluate patients’ foot wound healing,ischemia improvement,and infection control.Results:All patients were followed up.Except one case underwent amputation and two cases had a general curative effect,the postoperative curative effect of the other patients was significant,and the wound surface was gradually reduced until healing.The vascular index of dorsal metatarsal artery,blood flow of dorsal metatarsal artery,total blood flow of anterior tibial artery and posterior tibial artery increased one week and one month after surgery,and the differences were statistically significant compared with those before surgery(P<0.05).In WIFi grading,the wound grade decreased from the preoperative(2.05±0.78)grade to the(1.05±0.91)grade at the end of the migration period,the ischemic grade decreased from the preoperative(1.84±0.83)grade to the(1.47±0.84)level at the end of the migration period,and the infection grade decreased from the preoperative(1.89±0.66)level to the(1.11±0.88)level at the end of migration period,and the differences were statistically significant(P<0.05).Conclusion:The technique of tibial transverse transport can promote wound healing,and superb microvascular imaging can quantitatively monitor the hemodynamic changes in the process of bone transfer,which helps to clarify the mechanism of tibial transverse transport in the treatment of diabetic foot,and provides an important theoretical basis for the prognosis of the disease.
