Objective:Although thyroid surgery is a very common procedure, there is no well-developed method to assess its complications. This study aims to investigate the relationship between preoperative status and postoperative complications in patients undergoing thyroid surgery and to screen for important risk factors in order to predict the risk of postoperative complications by assessing the preoperative status of patients and to guide further treatment. Methods:Retrospectively analyze the preoperative status and the occurrence of postoperative complications in patients who attended the Department of Thyroid Surgery in First Hospital of Shanxi Medical University from October 2020 to September 2021 and were operated on by the same group of surgeons. The corresponding statistical methods were applied to screen the risk factors affecting the complications of thyroid surgery, and the corresponding regression models were finally established by multifactorial binary logistic regression. Results:A total of 864 patients were included according to the inclusion and exclusion criteria.Among them,259 patients developed complications. After corresponding statistical analysis and binary logistic regression analysis, the final variables found to be statistically significant in the regression model were: age, whether anemia, whether concomitant respiratory and digestive diseases, whether it was the first surgery, length of surgery, and extent of surgery(all P<0.05).The study showed that the risk of postoperative complications was higher in patients who were older, had concomitant anemia, respiratory and digestive disorders, were not first-time surgery, had a longer operative time, underwent total thyroidectomy, and had lateral neck dissection. The regression equation was obtained: logit(p)=-5.544 + 0.019(age)+ 0.781(anemia)+ 0.795(unilateral lobectomy)+ 2.893(total excision)+ 1.313(lateral neck lymph node dissection)+ 0.009(length of surgery)+ 1.709(non-first surgery)+ 1.606(respiratory disease)+ 0.791(digestive system disease). Conclusion:Through preliminary screening and statistics, this study obtained possible risk factors for postoperative complications in thyroid surgery and a preliminary risk assessment prediction model. With the help of such tools, clinicians can better manage the perioperative period and inform about the risks, thus reducing the chance of postoperative thyroid complications.
