ObjectiveTo explore the preventive effect of early rehabilitation activities on gastric retention in ICU patients with mechanical ventilation,in order to provide a new idea for clinical prevention of gastric retention.MethodPatients admitted to comprehensive ICU2 ward in a class A tertiary hospital in Nanchang city,Jiangxi Province from June 2021 to February 2022 were selected as the research subjects.Patients in comprehensive ICU2 ward were divided into experimental group(n = 44)and control group(n = 44)by coin toss method.The control group was given ICU routine rehabilitation nursing,and the experimental group was given ICU routine rehabilitation nursing + early rehabilitation activities.Baseline data,gastric retention,incidence of abdominal distention,aspiration,vomiting,and interrupted feeding,nutritional status,duration of mechanical ventilation,and length of ICU stay were collected in both groups.Results1.General data: A total of 88 subjects were included in this study,44 in each group.There was no statistical significance in gender,age,education level,APACHE Ⅱ score,BMI,disease diagnosis,enteral nutrition preparation,muscle strength and other baseline data between the two groups(P > 0.05).2.Gastric retention: compared with the two groups after intervention,the gastric retention in experimental group was less than that in control group[(95.70±26.06)ml VS(120.91±49.81)ml],the difference was statistically significant(P < 0.05).The amount of gastric retention in control group after intervention was more than before intervention [(120.91±49.81)ml VS(106.32±34.72)ml],and the difference was statistically significant(P < 0.05).The amount of gastric retention in experimental group after intervention was less than before intervention[(95.70±26.06)ml VS(103.86±30.47)ml],and the difference was statistically significant(P < 0.05).The difference of gastric retention between the experimental group and the control group before and after intervention was statistically significant(P < 0.05).3.Incidence of abdominal distention,aspiration,vomiting and interrupted feeding: Compared with the control group,the incidence of abdominal distention,aspiration and interrupted feeding in the experimental group was lower than that in the control group,and the difference was statistically significant(P < 0.05).There was no significant difference in the incidence of vomiting between 2 groups(P >0.05).4.Nutritional status: Comparison of nutritional parameters between the two groups after intervention showed that Hb,ALB and PA values in experimental group were higher than those in control group: Hb [(119.70±28.45)g/L VS(107.52±28.65)g/L],ALB [(35.15±5.31)g/L VS(32.43±6.1)g/L],PA [(0.18±0.06)g/L VS(0.15±0.05)g/L],The difference was statistically significant(P < 0.05).Hb,ALB and PA values of the experimental group after intervention were greater than before:Hb [(119.70±28.45)g/L VS(107.91±26.67)g/L],ALB [(35.15±5.31)g/L VS(32.13±6.09)g/L],PA [(0.18±0.06)g/L VS(0.16±0.06)g/L],The difference was statistically significant(P < 0.05).There was no significant difference in Hb,ALB and PA between the experimental group and the control group before and after intervention(P > 0.05).5.Duration of mechanical ventilation and length of ICU stay: Compared with the control group,the duration of mechanical ventilation in the experimental group was shorter than that in the control group [(16.41±2.76)days VS(18.05±4.32)days],and the length of ICU stay in the experimental group was shorter than that in the control group [(18.23±4.50)days VS(20.59±6.29)days],with statistically significant differences(P < 0.05).ConclusionEarly rehabilitation activities can prevent gastric retention in ICU patients with mechanical ventilation.Early rehabilitation activities can reduce the incidence of abdominal distention,aspiration and interrupted feeding,improve their nutritional status,and shorten their mechanical ventilation and ICU stay.
