Objective:Diabetes is an independent risk factor for heart failure.,and glycosylated hemoglobin(Hb A1c)levels are associated with heart failure. This study retrospectively analyzed the correlation between different Hb A1 c levels and heart failure markers and color doppler echocardiography indicators in diabetic patients with heart failure.glycosylated hemoglobin(Hb A1C)reflects blood sugar levels over the past two to three months. In this way,to investigate whether glycosylated hemoglobin can be a predictor of recent heart failure in diabetic patients. Methods:This study was a retrospective study.A total of 646 patients with diabetes complicated with heart failure who were hospitalized in the First Affiliated Hospital of Nanchang University from January 1,2015 to September 30,2019 were included as the clinical study object.Using the Kolmogorov-Smirnov test for normality of the data distribution. The patients were grouped according to the statistical interquartile range of Hb A1 C level. There were 187 patients with Hb A1 C value <6.5%;189patients with Hb A1 C value ≥6.5%-<7.5%;Hb A1 C value ≥7.5%-<8.5% of patients, total of 142 patients;Hb A1 C value greater than 8.5%,total of 128 patients. With Hb A1 C value <6.5% as the reference group, robust linear regression analysis was used to evaluate Hb A1 c level and heart failure indicators(such as NT-pro BNP,left ventricular end-diastolic size, left atrial diameter, left ventricular wall thickness, interventricular septal thickness, ejection score)relationship.Gender and age differences were additionally adjusted for in multivariate adjusted models. Results:Among the 4 groups of patients with different Hb A1 C levels, after adjusting for gender and age differences, take Hb A1 C value <6.5% as reference value, with the increase of glycosylated hemoglobin levels, the relative value of NT-pro BNP in the study subjects were 590.79,1019.35,and 1765.65,showing an linear upward trend. For every unit of increase in glycosylated hemoglobin, the NT-pro BNP value increased by 389.82 [14.51,765.12],and between glycosylated hemoglobin and NT-pro BNP have a significant correlation(P < 0.05).It was no significant correlation between the level of glycosylated hemoglobin and left ventricular end-diastolic diameter, left atrial diameter, left ventricular wall thickness, interventricular septum thickness and ejection fraction(P > 0.05).Conclusion:With the increase of glycosylated hemoglobin level,NT-pro BNP in diabetic patients with heart failure gradually increased, showing a positive correlation. Is a risk factor for predicting recent heart failure in diabetic patients. However, there was no significant correlation between left ventricular end-diastolic diameter, left atrial diameter, left ventricular wall thickness, interventricular septal thickness, and ejection fraction and glycosylated hemoglobin.
