ObjectiveTo explore the intervention effect of nurse-led network group brief behavioral therapy on insomnia,anxiety and depression in patients with breast cancer survivors,in order to provide a more efficient and convenient way to improve the physical and mental symptoms of insomnia patients with breast cancer.MethodsFrom December 2020 to May 2021,breast cancer survivors in the breast surgery department of a grade A hospital in Zhengzhou were selected as the investigation subjects.The Insomnia Severity Index(ISI)was distributed to patients who complained of poor sleep quality.The intervention group and the control group were determined by a coin toss with 40 patients in each group..Before intervention,general data questionnaire,Pittsburgh Sleep Quality Index(PSQI),Self-rating Anxiety Scale(SAS)and self-rating Depression Scale(SDS).The intervention lasted for 7 weeks,once a week.In the first week,the two groups received online health education through Tencent conference,including common factors of insomnia in breast cancer patients and sleep hygiene education.The intervention was performed once a week for about90 minutes for 6 weeks,the control group regularly pushed popular science texts and videos related to sleep hygiene through wechat group every week.On the basis of the control group,the intervention group received networked group brief behavioral treatment for insomnia for insomnia through Tencent video conference.PSQI,SAS and SDS were used to evaluate the efficacy after intervention and three months after intervention.Results1.Comparison of baseline data between the two groups: Before intervention,there were no significant differences in the general information of marital status,cultural status,religious belief,long-term residence,PSQI total score and scores of various dimensions,SAS scores,SDS scores,and incidence of anxiety and depression between the two groups(P > 0.05).2.Comparison of sleep status between the two groups after intervention and three months after intervention:(1)Comparison between groups showed that the total score of PSQI and the scores of all dimensions in the intervention group were lower than those in the control group after intervention,and the differences were statistically significant except for the sleep disorder dimension(P < 0.05).Three months after intervention,the PSQI total score and scores of all dimensions in the intervention group were lower than those in the control group,and the differences were statistically significant except for sleep disorder and daytime dysfunction(P <0.05).(2)Friedman test results showed that there were statistical differences in PSQI total score and scores of all dimensions between the two groups(P < 0.05).(3)Pair-to-side comparison of PSQI scores at different measurement points showed that after intervention,the PSQI total score and scores of all dimensions in the two groups were significantly lower than before intervention,and the differences were statistically significant except for sleep time and sleep efficiency in the control group(P < 0.0167).Three months after intervention,the total score of PSQI and scores of all dimensions in 2 groups were significantly lower than before intervention,and the differences were statistically significant except for the sleep quality dimension,sleep time dimension,sleep time dimension and sleep efficiency dimension of patients in the control group(P < 0.0167).(4)After the intervention,30 patients(85.71%)in the intervention group were effectively treated for insomnia,while 15 patients(39.47%)in the control group were effectively treated,the difference was statistically significant(P < 0.05).Three months after intervention,27 cases(77.15%)in the intervention group were effectively treated for insomnia,while 10 cases(26.32%)in the control group were effectively treated,with statistical significance(P < 0.05).3.Comparison of anxiety and depression between the two groups after intervention and three months after intervention:(1)Comparison between groups showed that the scores of anxiety and depression in the intervention group were lower than those in the control group after intervention and three months after intervention,with statistical significance(P < 0.05).(2)Repeated measures analysis of variance showed that time effect,inter-group effect and interaction effects on SAS were statistically significant(P < 0.05).SDS scores of the two groups showed significant difference in time effec and interaction effect(P<0.05),but no significant difference in inter-group effect(P > 0.05).(3)Pair-to-side comparison of SAS and SDS scores at different measurement points showed that SAS and SDS scores before intervention and after intervention were statistically different between two groups(P < 0.05).SAS and SDS scores of the intervention group were statistically different before intervention compared with three months after intervention(P < 0.05).SAS scores in the intervention group were statistically different from those three months after intervention(P < 0.05).(4)After intervention,the incidence of anxiety and depression in the intervention group(25.71% and 34.29%)was significantly lower than that in the control group(57.90% and 44.74%),but only the incidence of anxiety was statistically different(P < 0.05).Three months after intervention,there was no significant difference in the incidence of anxiety and depression between the intervention group and the control group(P > 0.05).Conclusions1.Networked group brief behavioral treatment can effectively improve insomnia symptoms in breast cancer survivors for at least 3 months.2.Network group brief behavior treatment can reduce anxiety and depression level of breast cancer survivors to a certain extent.
