Objective The purpose of this study was to observe the expression levels of C-type lectin like receptor 2(CLEC-2),lipoprotein associated phospholipase A2(Lp-PLA2)and highsensitivity C-reactive protein(hs-CRP)in the plasma of patients with acute coronary syndrome(ACS),and to explore the diagnostic value of the three in ACS,and to explore the correlation between the three and the severity of coronary artery disease through the number of coronary artery lesions and Gensini score,In order to provide high sensitivity and high specificity markers for the diagnosis of ACS and the judgment of the severity of coronary artery disease.Methods 90 inpatients in the Department of cardiovascular medicine of the First Affiliated Hospital of Jiamusi University from November 2020 to October 2021 were selected.According to the clinical diagnostic criteria,they were divided into 30 cases of stable angina pectoris(SAP)and 60 cases of acute coronary syndrome(ACS).At the same time,30 hospitalized patients without obvious stenosis(stenosis < 50%)were selected as the control group.The age,sex and other general data of the selected patients and the results of clinical biochemical indexes were recorded.All the selected patients underwent percutaneous coronary angiography.According to the number of coronary artery lesions,the patients in ACS group were divided into 23 cases in single vessel group,23 cases in double vessel group and 14 cases in three vessel group.According to Gensini score,they were divided into 15 cases in low group(≤ 21 points),26 cases in medium group(22-52 points)and 19 cases in high group(≥ 53 points).The levels of CLEC-2 and Lp-PLA2 were measured by enzyme-linked immunosorbent assay,The level of hs-CRP was measured by immune scattering turbidimetry,and the experimental data were statistically analyzed.Results 1.The levels of plasma CLEC-2,Lp-PLA2 and hs-CRP in ACS group were significantly higher than those in control group and SAP group(P < 0.05).Multivariate logistic regression analysis showed that CLEC-2,Lp-PLA2 and hs-CRP were independent risk factors for ACS.The area under the ROC curve of plasma CLEC-2 in the diagnosis of ACS was 0.925(P < 0.05).When the cut-off value of CLEC-2 was 258.8,the sensitivity and specificity were86.67% and 81.67% respectively.The area under the ROC curve of plasma Lp-PLA2 in the diagnosis of ACS was 0.833(P < 0.05).When the cutoff value of Lp-PLA2 was 189.5,the sensitivity and specificity were 86.67% and 68.33% respectively.The area under the ROC curve of plasma hs-CRP in the diagnosis of ACS was 0.941(P < 0.05).When the cut-off value of hs-CRP was 5.28,the sensitivity and specificity were 85.00% and 86.67% respectively.The area under ROC curve was 0.987(P < 0.05),the sensitivity was 95.00%,and the specificity was95.83%.2.ACS group was divided into three subgroups according to the number of coronary artery lesions(single vessel group,double vessel group and three vessel group).There was no significant correlation between the levels of plasma CLEC-2,Lp-PLA2 and hs-CRP and the number of coronary artery lesions.3.ACS group was divided into three subgroups according to Gensini score(low group,medium group and high group).The levels of CLEC-2,Lp-PLA2 and hs-CRP in high group were significantly higher than those in low group and medium group,and the levels of CLEC-2,Lp-PLA2 and hs-CRP were positively correlated with Gensini score.Conclusions 1.The levels of CLEC-2,Lp-PLA2 and hs-CRP in patients with ACS were significantly increased,and CLEC-2,Lp-PLA2 and hs-CRP were independent risk factors of ACS.Combined detection of CLEC-2,Lp-PLA2 and hs-CRP had higher diagnostic efficiency in the diagnosis of ACS.2.There was no significant correlation between the levels of CLEC-2,Lp-PLA2 and hs-CRP and the number of coronary artery lesions.3.The levels of CLEC-2,LpPLA2 and hs-CRP were positively correlated with the severity of coronary artery disease in patients with ACS. |
