Objective : To explore the clinical effect and safety of spraying irrigation therapy--Kaihoujian Spray(children type)oral local spray combined with Traditional Chinese medicine enema in the treatment of common hand-foot-mouth disease(HFMD),in order to provide a basis for the application of Traditional Chinese medicine in children with common HFMD and to optimize the way of administration of HFMD.Methods : A total of 90 children with common HFMD aged 1~5 years who were hospitalized in the Department of Infectious Diseases of our hospital were selected as the research subjects which from January 2021 to June 2021.they were randomly divided into three groups according to the hospitalization,with 30 cases in each group.Kaihoujian Spray(children type)spray combined with Traditional Chinese medicine enema group(the combined group for short),Kaihoujian Spray(children type)group(the Kaihoujian group for short)and symptomatic supportive treatment group(the control group for short).The combined group was treated with Kaihoujian Spray(children type)spray combined with traditional Chinese medicine enema.The Kaihoujian group was given symptomatic treatment and added to open throat sword spray roar.The control group was given symptomatic support treatment;The treatment course was 3~5 days.The general information,fever reduction time,rash/herpes regression time,oral ulcer healing time,complete disappearance of clinical symptoms and signs,incidence of clinical adverse reactions,severe transformation,blood WBC,IL-6,IL-10,TNF-α levels before and after treatment,length of hospital stay and hospitalization expenses of the three groups were compared.Result:1.The general data of the three groups: gender,age and admission temperature had no statistical significance(P>0.05),they were comparable with each other.2.The antipyretic time(hours)in the three groups: there was statistical significance in the time of fever reduction among the three groups(P<0.05).After pair-pair-comparison,the time of antipyretic in the combined group compared with the control group and the Kaihoujian group were more shorter,and the difference was statistically significant(F=45.19,P=0.001,0.001).Compared with the control group and the Kaihoujian group,the difference was not statistically significant(F=45.19,P=1.000).3.The time(hours)of rash/herpes resolution in the three groups: there was statistical significance in the time of rash/herpes regression among the three groups(P<0.05).The time of rash/herpes regression in the combined group was shorter than that in the control group and the Kaihoujian group,and the difference was statistically significant(F=44.09,P=0.000,0.016).The time of rash/herpes regression in the Kaihoujian group compared with the control group was more shorter,and the difference was statistically significant(F=44.09,P=0.000).4.The healing time of oral ulcer(hours)in the three groups: there was statistical significance in the healing time of oral ulcer among the three groups(P<0.05).After pairings,oral ulcer healing time in the combined group compared with the control group and the Kaihoujian group were more shorter,the difference was statistically significant(F=70.31,P=0.000,0.034).The healing time of oral ulcer in the Kaihoujian group was shorter than that in the control group(F=70.31,P=0.000).5.The time of complete disappearance of clinical symptoms and signs(hours)in the three groups: the time of complete disappearance of clinical symptoms and signs was statistically significant among the three groups(P<0.05).Then pair-up comparison showed that the time of complete disappearance of clinical symptoms and signs in the combined group compared with the control group and the Kaihoujian group was more shorter,and the difference was statistically significant(Z=6.635,2.271,P=0.000,0.000).Kaihoujian group compared with the control group,there was no statistical significance(Z=1.703,P=0.267).6.Incidence of adverse reactions in three groups(vomiting,diarrhea,rash,abnormal liver and kidney function)was no statistical significance(χ2=0.200,P =0.905).7.There were no severe cases in the three groups.8.The levels of blood WBC,IL-6,IL-10 and TNF-α in 3 groups before treatment was no statistical significance(P>0.05).between the three groups,there were statistically significant differences after treatment and before treatment,and there was a significant decrease(P<0.05).After treatment,the levels of blood WBC,IL-6,IL-10 and TNF-α in the combined group compared with the control group and the Kaihoujian group were more lower,there were statistically significant differences(P < 0.05).There was no significant difference in blood WBC level between the Kaihoujian group and the control group(P>0.05),while the levels of IL-6,IL-10 and TNF-α in the Kaihoujian group compared with the control group were more lower,there were statistically significant differences(P<0.05).9.The length of stay(days)and cost of hospitalization(yuan)in the three groups were statistically significant differences(P<0.05).After pairwise comparison,the length of hospital stay in the combined group was less than that in the Kaihoujian group and the control group,and the difference was statistically significant(F=15.52,P=0.021,0.000).The length of hospital stay in the Kaihoujian group compared with the control group was more less,there was statistically significant difference(F=15.52,P=0.011).The hospitalization cost of combined group was less than that of Kaihoujian group and control group,and the difference was statistically significant(Z=2.431,P=0.012,0.000).The hospitalization cost of the Kaihoujian group compared with the control group was more less,there was statistically significant difference(Z=2.431,P=0.010).Conclusion:Kaihoujian Spray(children type)spray combined with Traditional Chinese medicine enema for the treatment of common hand-foot-mouth disease administration method is simple,can obtain ideal treatment effect,to shorten the time and hospital stay in children with symptoms subsided,significantly reduce medical costs,at the same time can effectively regulate the body’s inflammatory factor,the security is good,suitable for clinical use.
