Objective:Through literature review and expert letter consultation,a chest physical therapy rehabilitation program was established to compare the effects of chest physical therapy rehabilitation program and routine ICU nursing measures for PATIENTS with mechanical ventilation in ICU.To investigate the effects of chest physical therapy and rehabilitation program on the incidence of VAP,length of ICU stay,duration of mechanical ventilation and OI in ICU patients with mechanical ventilation,so as to provide reference for future clinical research.Method:This study was divided into two stages: the construction of chest physiotherapy rehabilitation program for ICU mechanical ventilation patients and the intervention of chest physiotherapy rehabilitation program.1.Construction of chest physical therapy rehabilitation program: Through the chest physical therapy of related literatures,mechanical ventilation in patients with chest physical therapy rehabilitation plan formulation,through to the 18 domestic relevant clinical nursing,clinical medicine,rehabilitation medicine and Delphi expert enquiry for nursing management experts,according to the experts put forward relevant advice to modify final ICU mechanical ventilation in patients with chest physical therapy rehabilitation program.2.Chest physical therapy intervention study: In this study,32 patients who received mechanical ventilation and routine CARE in ICU from January 2021 to June 2021 were selected as the control group according to the time order of admission in a grade III,Grade A general hospital in Nanchang.Thirty-two patients who received mechanical ventilation and chest physical therapy from July 2021 to December 2021 were selected as the intervention group of this study.The general data of the two groups before intervention were analyzed and compared,and the incidence of VAP,oxygenation index mechanical ventilation time and length of ICU stay in the two groups were analyzed and compared.Result:1.General data A total of 64 patients were included in this study,including the control group(n=32 cases)and the intervention group(n=32 cases).There was no statistically significant difference in the general data of the two groups(P > 0.05).2.Compared with the incidence of VAP in the OI groups,the incidence of VAP in the control group(n=32 cases)was 14(43.75%),and that in the intervention group(n=32 cases)was 4(12.5%),the difference was statistically significant(P < 0.05).There was no significant difference in OI between the control group and the intervention group before intervention [(182.84±8.28)mm Hg VS(181.41±7.54)mm Hg](P > 0.05).The comparison of intervention for 3,5 and 7 days was as follows:[(212.84±8.28)mm Hg] VS(245.94±11.82)mm Hg,[(240.03±18.93)mm Hg VS(265.66±12.98)mm Hg],[(271.25±9.43)mm Hg VS(318.53±15.74)mm Hg],the differences were statistically significant(P < 0.05).3.The results of mechanical ventilation time and ICU stay in the control group and the intervention group were [(12.25±1.87)days VS(10.63±1.95)days] and[(14.66±3.35)days VS(12.94±2.59)days,respectively.There was statistical significance(P < 0.05).Conclusion:1.The chest physical therapy and rehabilitation program for ICU mechanical ventilation patients established by Delphi method is innovative,feasible and practical by searching relevant databases at home and abroad,consulting relevant literature and establishing research teams.2.Chest physical therapy and rehabilitation programs can effectively prevent or reduce the incidence of VAP,improve the OI of patients with mechanical ventilation,and shorten the mechanical ventilation time and ICU stay of severe patients.
