Objective:The purpose of this study to investigate the correlation between NLR,PLR,LMR and renal injury in children with Henoch-Sch(?)nlein purpura and the predictive value of early in children with Henoch-Sch(?)nlein purpura renal injury.Methods: 180 children with Henoch-Sch(?)nlein purpura treated in Chaohu Hospital of Anhui Medical University from June 2016 to June 2021 were included in the study.They were divided into renal injury group(n=30)and non renal injury group(n=150).Collect general clinical data(such as age,sex,season,etc.)and relevant laboratory indicators after admission,including blood biochemical indicators: high-sensitivity C-reactive protein,creatinine,albumin and blood routine related indexes: leukocyte count,neutrophil count,lymphocyte count,monocyte count,platelet count,and calculate Neutrophil/lymphocyte ratio,platelet/lymphocyte ratio and lymphocyte/monocyte ratio.Binary logistic regression was used to analyze the correlation between it and renal injury of Henoch-Sch(?)nlein purpura,and ROC curve was drawn to analyze the predictive value of each index in renal injury of Henoch-Sch(?)nlein purpura in children.Results:(1)There was no significant difference in age and gender between Henoch-Sch(?)nlein purpura renal injury group and non renal injury group(P>0.05).(2)In the comparison of serum related parameters between the two groups,WBC,NEU,LYM,MONO,PLT,NLR,PLR and Cys C in the renal injury group were higher than those in the non renal injury group,while LYM and LMR were lower than those in the non renal injury group,the difference was statistically significant(p<0.05),while there was no significant difference in hs-CRP,CAEA,BUN and ALB between the two groups(P > 0.05).(3)The results of binary logistic regression analysis showed that NLR(OR=1.723,P=0.015),PLR(OR=1.009,P=0.020)and Cys C(OR=1.036,P=0.017)were the risk factors for renal injury caused by Henoch-Schonlein purpura,and LMR(OR=0.680,P=0.012)was the protective factor for renal injury caused by Henoch-Sch(?)nlein purpura.(4)ROC curve analysis showed that the AUCs detected by NLR,PLR,LMR and their combination were 0.831,0.809,0.785 and 0.846 respectively.According to the AUC size,NLR was the single best predictor of HSP renal injury,with specificity of 72.7%and sensitivity of 80.0%.The optimal critical value is 2.48.The combined detection value of the three indexes was the highest,with a sensitivity of 73.3% and specificity of 84%.Conclusion:NLR,PLR and LMR are significantly correlated with renal injury in HenochSch(?)nlein purpura.NLR and PLR are independent risk factors for renal injury,and LMR is a protective factor for renal injury.Among them,NLR is the best factor for diagnosing renal injury in children with HSP,which has a certain potential value for early prediction of renal injury in Henoch-Sch(?)nlein purpura.When predicting whether there is renal injury in Henoch-Sch(?)nlein purpura,the combined detection of the three indexes is more significant than the single index of NLR,PLR and LMR.
