| Objective: To observe the effect of 4 weeks’ Isokinetic Eccentric Exercise combined with Floss Band on patients after reconstruction Anterior Cruciate Ligament for 6 months,to observe the effect of postoperative rehabilitation.So as to investigate the effects of knee extension muscle strength,explosive power,endurance,muscle circumference,knee range of motion,knee stability,proprioception and knee subjective function.Methods: Twenty patients with ACLR for 6 months after surgery were selected for rehabilitation training twice a week for 4 weeks.Extract the basic information of the subjects,and then group them by random sampling: Group IE and Group IE+FB.Training speed,frequency,duration and location are all the same.Patients were instructed to perform simple warm-up activities for 5 mins before training;After the preparation,the IE+FB group used Iso Med X-Series 3500 isometric muscle strength training system to conduct isometric centrifugal training for the patients.When the affected knee was bent at 90°,the therapist carried out 50% tension spiral around the affected leg from 1-2cm above the iliac to the greater trochanter.Quadriceps femoris isokinetic CENTRIFUgation(ISokinetic M1 ECC,M1 10 cm/s and M2 20 cm/s in M2 CON mode)was performed for 10 times/group.The 10 groups were completed,and the rest between the groups was 1 min.Floss Band was removed between the groups.Ten groups were treated with ice for 5 mins.The IE group completed the training without winding the Floss Band under the same condition.Pain should be controlled during the whole training process.According to the pain numerical scoring method,if the patient has pain more than 3 points,the training should be stopped immediately.Above is a complete training process.Patients were assessed for motor function before and after intervention,and the main assessment indicators included:Peak Torque,Peak Torque ratio between the affected side and the healthy side,Peak Torque ratio to body Weight,explosive force and endurance of knee extension under Isomed 2000 multi-joint isometric muscle force testing system;Dynamic stability evaluation of the Knee joint under opti-Knee 3d motion analysis system;Evaluation of balance Average Trace Error(ATE)in Tecno Body Balance and Proprioception Test and Training System(PK254);The circumference Of 5cm and 10 cm and Knee Range Of Motion(KROM)in the suprapatellar region;Lysholm Knee Scoring Scale(LKSS);2000 IKDC subjective evaluation table of knee joint and Restore satisfaction score..Data were collected by Excel and analyzed by SPSS 19.0.Results: After the subjects in both groups completed the 4-week training,the data analysis results were as follows:1.Thigh circumference: intra-group comparison: Compared with before training,thigh circumference of the affected side(5cm and 10 cm above the iliac)in IE+FB group increased significantly after training,and the difference was statistically significant(P < 0.01);The thigh circumference of the affected side(10cm above the patellar)increased in IE group after training,and the difference was statistically significant(P< 0.05).Comparison between groups: after 4 weeks of training,there was no significant difference in the increase of affected thigh circumference(5cm,10cm)between the two groups(P > 0.05).2.Strength quality: Intra-group comparison: Compared with before training,PT and PT/BW of knee extension on the affected side in IE+FB group increased significantly after training,with extremely significant statistical difference(P <0.001);The explosive force of knee extension increased significantly,with significant statistical difference(P < 0.01);SI increased significantly with statistical difference(P< 0.05);There was no significant increase in endurance and no statistical difference(P > 0.05).Comparison between groups: After 4 weeks of training,the increase of PT,PT/BW and SI in the IE+FB group was greater than that in the IE group(P < 0.05),but there was no significant difference in the increase of endurance and explosive power between the two groups(P > 0.05).3.KROM: Intra-group comparison: compared with before training,there was no statistically significant difference in KROM of the affected side between the two groups after training(P > 0.05).Inter-group comparison: after 4 weeks of training,the KROM of the affected side increased in the two groups,and the difference was not statistically significant(P > 0.05).4.Balance and proprioception: intra-group comparison: compared with before training,there was no statistical difference in ATE and dynamic stability score of knee joint in IE+FB group after training(P > 0.05);ATE of the affected side was significantly different in IE group after training(P < 0.05).Comparison between groups: after 4 weeks of training,there was no statistical difference in the relative changes of ATE and dynamic stability score of knee joint between the two groups(P > 0.05).5.Subjective function of knee joint: intra-group comparison: Compared with before training,IKDC and satisfaction score of motor ability recovery in IE+FB group were significantly different after training(P < 0.01),and LKSS score was significantly different(P < 0.05).IKDC score of IE group was significantly different after training(P < 0.05).Comparison between groups: after 4 weeks of training,there was no significant difference in IKDC,LKSS and satisfaction score of motor ability recovery between the two groups(P > 0.05).Main conclusions:1.4 weeks of constant speed centrifugal combined with quadriceps voodoo belt with 6 months after training can effectively increase the ACLR with lateral thigh circumference degree,and muscle strength and explosive force,bilateral symmetry index and strength,and to stretch the knee muscle strength,power,and power on both sides symmetry index to improve the effect of the quadriceps muscle and is superior to the conventional constant speed centrifugal training group;2.4 Quadriceps femoris isokinocentrifugation combined with voodo belt training for weeks can effectively improve subjective function of knee joint and self-perception of patients 6 months after ACLR surgery;3.4 weeks of constant speed centrifugal combined with quadriceps voodoo with training to improve the ACLR six months postoperative patients with lateral muscular endurance,knee joint mobility,balance and proprioception effect is not obvious,and the quadriceps muscle and conventional constant speed centrifugal training on the improvement of balance and proprioception is superior to constant speed centrifugal combined with quadriceps voodoo with training. |
