Literature And Clinical Study On The Treatment Of Some Respiratory Diseases With Pungent Dispersion Bitter Purgation Method

Purpose:The method of Pungent dispersion bitter purgation method is a unique traditional Chinese medicine treatment based on odor classification.Many warm pathologists use it for the syndrome of middle energizer.It is mainly used in digestive system diseases,involving the treatment of some cardiovascular diseases and diabetes.Through years of study with teachers,the author found that tutors applied this method to respiratory diseases such as cough,asthma and COPD,and achieved good clinical efficacy.Many literature reports also confirmed that this method had good clinical efficacy in the treatment of respiratory diseases.In view of the lack of a unified understanding of the Pungent dispersion bitter purgation method,there is a lack of systematic review of the application of this method in the treatment of respiratory diseases.Therefore,this paper summarizes the Pungent dispersion bitter purgation method from the literature research,clarifies its concept,theoretical connotation and therapeutic mechanism,and expounds and summarizes Pungent dispersion bitter purgation method in the treatment of respiratory diseases by combining the therapeutic mechanism with the pathogenesis of the disease.Through clinical research,the clinical experience of the tutor in the treatment of respiratory diseases is summarized,and some theories are verified and deepened,in order to provide reference for clinical treatment of respiratory diseases and improve clinical efficacy.Method:1.Literature research method: by using the method of literature research,the ancient masterpieces such as "Huangdi Nei Jing","Treatise on Febrile Diseases","Synopsis of Golden Chamber","Treatise on Febrile Diseases","Plain Questions Mystery Original","Medicinal Fa Xiang","Dan Xi Heart Method","Clinical Guideline Medical Records" and "Warm Diseases Identification" were selected as the research objects,and the modern literature such as "Warm Disease Prescriptions and Syndromes and Miscellaneous Diseases Identification and Treatment","Zang Xiang Theory and Diagnosis and Application-Spleen,Liver,Lung" and "Asthma album" were selected as the research objects.The concept,theoretical connotation,therapeutic mechanism and pathogenesis of respiratory diseases were sorted out.Based on the combination of therapeutic mechanism and disease pathogenesis,this paper summarizes the treatment of respiratory diseases by Pungent dispersion bitter purgation method,and summarizes its main syndrome types.2.Clinical research methods: from the electronic medical record database of Qi Huang TCM outpatient department from 2011 to 2021,the medical record data of Associate Professor Shi Qiang using Pungent dispersion bitter purgation method to treat respiratory diseases were selected as the research object,and the effective medical records were extracted in strict accordance with the inclusion and exclusion criteria.The statistical description method was used to analyze the medical record data,and the main causes,locations,pathogenesis,main pulse conditions,tongue conditions,clinical symptoms and commonly used drugs involved were analyzed.To summarize the common syndrome types of Professor Shi Qiang in treating respiratory diseases with Pungent dispersion bitter purgation method.3.Combined with literature research and clinical research results are described.The former plays a supplementary role in guiding the latter,the latter plays a verification role in deepening the former,better combination of theory and experience,play the research value of the two.Results:1.Literature research results:(1)The concept of Pungent dispersion bitter purgation method: The Pungent dispersion bitter purgation method has broad and narrow sense.The generalized method of Pungent dispersion bitter purgation refers to the combination of pungent drugs and bitter drugs to regulate Qi.In a narrow sense,the method of Pungent dispersion bitter purgation refers to a therapeutic method for regulating qi activity by combining pungent warm herbs with bitter cold herbs.(2)The therapeutic mechanism of the method of Pungent dispersion bitter purgation: the combination of pungent and pungent,cold and heat,can achieve the effect of opening and resolving phlegm(wet),clearing heat,dredging qi,regulating spleen and stomach and triple energizer,harmonizing cold and heat,clearing heat.(3)The pathogenesis of most respiratory diseases is consistent with the therapeutic mechanism of the Pungent dispersion bitter purgation method: the lung is the viscera of the pivot,and its function depends on the lung qi.Therefore,the pathogenesis of respiratory diseases is(1)various factors leading to lung failure;(2)Most of them are inhibited by evil,and the rise and fall are inappropriate;(3)involving triple energizer(the upper warmer is indigenous);(4)easy to evolve into damp heat,phlegm heat,drink heat,water fire and other two evils with the state.(4)The main syndrome types of Pungent dispersion bitter purgation method in the treatment of respiratory diseases :(1)Cold drink stagnation of lung,phlegm throat;(2)exterior cold drinks,lung obstructs qi and heat;(3)Drink evil heat,lung depression yang;(4)drink heat forced lung,lung back;(5)exogenous wind cold,phlegm heat lung;(6)Phlegm heat junction,lung qi stagnation;(7)phlegmheat obstructing the lung,and lung obstruction and blood stasis;(8)wind and cold attacking the surface,upper Jiao stagnation;(9)Dampheat stagnation,middle and upper mass.2.Clinical research results:Among 2352 effective cases diagnosed as "cough" and 88 effective cases diagnosed as "asthma",it is found through statistical description:(1)Cough and asthma treated with Xin Kai Ku Jiang method were more common in children under 9 years old,and more common in men than women;(2)The main Western medicine diseases involved in the treatment of cough by Pungent dispersion bitter purgation method are upper respiratory tract infection,bronchitis,pharyngitis,eosinophilic cough,cough variant asthma,chlamydia infection and mycoplasma infection.The main diseases are Yang Ming,Tai Yin and Shao Yang.The main cause of the disease is wet,wind,heat,phlegm,drink evil.The main pathogenesis is depression,deficiency and deficiency.Common pulse is floating pulse,string pulse,not fluent pulse.Common tongue is light red,sharp red,red,partial red.Common tongue coating is yellow,yellow slightly thick greasy,thin white.Common clinical symptoms include cough,nasal congestion,runny nose,yellow sputum,nausea or vomiting,molars,foreign body sensation of throat,and heart distension.(3)The main western medicine disease involved in the treatment of asthma with Pungent dispersion bitter purgation method is bronchial asthma.The main diseases are Yang Ming,Tai Yin and Shao Yang.The main cause of the disease is wet,wind,heat,phlegm,drink evil.The main pathogenesis is depression,deficiency and deficiency.Common pulse is floating pulse,string pulse,not fluent pulse.Common tongue is light red,sharp red,red,partial red.Common tongue coating is yellow,yellow slightly thick greasy,floating yellow.Common clinical symptoms include cough,wheezing,chest tightness,chest obstruction,heart distension,nausea,vomiting,and molars,etc.(4)The main types of syndrome and treatment of cough and asthma with Pungent dispersion bitter purgation method:(1)Shao Yang(upper warmer)Tai Yin(lung)damp evil heat;(2)middle warmer(spleen and stomach)damp heat,Tai Yin(lung)phlegm heat;(3)Tai Yin(lung)phlegm dampness heat,Yang Ming(large intestine)Qi stagnation;(4)Yang Ming(stomach)drink evil heat,spread to Tai Yin(lung);(5)Shao Yang(upper warmer)drink evil heat,forced by Tai Yin(lung);(6)Tai Yin(lung)cold drink heat,thrust throat.Conclusion:⒈ The method of Pungent dispersion bitter purgation is a therapeutic method for regulating qi movement by using herbs with pungent taste divergence and bitter descending.It can open phlegm(wet),drink heat,dredge qi,regulate spleen and stomach and triple energizer,reconcile cold and heat,clear heat.Hard to divergent,open,bitter to vent,both have aggressive nature,hard to open and bitter law is the method of treatment of excess,deficiency syndrome should not be used,mixed with deficiency and excess should be used with complementary method.⒉ The main pathogenesis of respiratory diseases such as cough and asthma is various factors leading to lung failure.Among them,the method of Pungent dispersion bitter purgation is mainly aimed at the respiratory disease syndrome with the main contradiction of evil qi repression and lung disillusionment.It has good curative effect on the respiratory disease syndrome that the disease develops into the condition of damp heat,phlegm heat,drinking heat,water and fire.⒊ The applicable syndrome types of Pungent dispersion bitter purgation method in the treatment of respiratory diseases are :(1)Shao Yang(upper warmer)Tai Yin(lung)damp evil heat;(2)middle warmer(spleen and stomach)damp heat,Tai Yin(lung)phlegm heat;(3)Tai Yin(lung)phlegm dampness heat,Yang Ming(large intestine)Qi stagnation;(4)Yang Ming(stomach)drink evil heat,spread to Tai Yin(lung);(5)Shao Yang(upper warmer)drink evil heat,forced by Tai Yin(lung);(6)Tai Yin(lung)cold drink heat,thrust throat.
