Research On The Optimization Of Family Doctor Contract Service In D City

In April 2019,the General Office of National Health Commission issued the Notice on Completing the Contracted Services of Family Doctor in 2019,enrich the connotation of their services,increase the attractiveness of their services,pay attention to performance,quality and service perception,and continuously improve the quality of contracted service.As a county-level city in developed areas,city D of Jiangsu Province has always attached importance to people’s health,With the development of family doctor contract services,further promoted the development of basic medical services,basic public health services,and constantly improved residents’ health literacy and sense of contract acquisition.However,in the practice of family doctor signing service,some problems have arisen,such as the ineffective use of referral service of family doctor signing service,the low overall cognition and satisfaction of the contracted residents to the family doctors,the weak service ability of the family doctor team,the relative lack of professionals,and the failure to change the health concept in time.It is necessary to innovate the theories of family doctor service,to guide the practice,so as to realize the innovation practice.This paper makes an in-depth exploration and analysis of the problems and effects during the practice of family doctor contract service in D city,so as to optimize the contract service and provide a basis for relevant departments to formulate relevant policies.Firstly,through the method of literature research and with the help of multiple literature research platforms,this paper mainly selects Chinese journals and some foreign literature from 2014 to 2021 to sort out,so as to understand the classical theories and the latest trends,and find scientific and effective methods for the Countermeasures of signing service for family doctors in D city;Secondly,the expert interview method,which invites various authorities and parties in D city to conduct face-to-face interviews to understand their views on the contracted service work of family doctors.According to the interview results,a preliminary questionnaire was designed,and revised through the pre-survey.Thirdly,multi-stage stratified random sampling,City D was divided into three layers according to urban areas,urban and rural areas,and rural areas.One street(township)was randomly selected from each layer as the sample area.Then,150 people(urban area),50 people(urban and rural area)and 50 people(rural area)were randomly selected from each sample area.A total of 250 people were selected as the survey object for questionnaire survey.And taking 17 primary medical institutions in D city as the scope,3 family doctor teams were randomly selected from 9 primary medical institution,204 people in total,questionnaire survey was conducted one by one.Finally,Conduct statistical analysis on the survey data,find problems and explore the outcome plan.On the basis of introducing the current situation of family doctor team construction,contracted service and contracted residents’ satisfaction in D city,this paper deeply analyzes the problems existing in the contracted family doctor service in D city: thereferral of the contracted service is not used effectively,the overall cognition degree and satisfaction degree of the residents with contracted services are lower,the family doctor team is weak in service ability,lacking in professional talents and centering on disease.The reasons are as follows: the imperfect multi department coordination mechanism,the implementation of hierarchical diagnosis and treatment system is not in place,policy publicity is not in place,level of information construction is not high,talent training mechanism is not perfect,talent welfare is low and concept of health has not changed in time.Five scientific,effective and practical improvement measures were proposed through research,discovery of problems and exploration of their causes,combined with the scientific methods summarized in the literature and the actual situation of family doctor contract service in D City.Respectively: establishing and improving the mechanism for coordination between various departments,strengthening the publicity and information construction of family doctor contract service,optimizing the talent training incentive mechanism,improve the performance incentive and restraint mechanism and promote the establishment of scientific and healthy concept.
