Purpose:Based on the interdisciplinary research foundation of society、physiology and psychology,this study introduces the allostatic load(AL),which is a chronic stress model that reflects the abrasion and consumption of the brain and physiological system caused by continuous stress stimulation.The study used the data of “China Health and Retirement Longitudinal Study(CHARLS)” in 2015 to explore the physiological and pathological factors of multi-dimensional health conditions in the middle-aged and elderly from a multi-system point of view,to verify whether the allostatic load index of the multi-biological system in the middle-aged and elderly is a relatively independent risk factor for multi-dimensional health(the main research);further combined with the CHARLS 2018,to explore the correlation between AL and multi-dimensional health changes in the middle-aged and elderly(extended research).Provide certain social medical evidence for the implementation of health management of middle-aged and elderly groups and the formulation of relevant health policies.Methods:This study makes full use of the CHARLS physical examination and blood test data.Firstly,which was descriptive analysis of the overall variables;secondly,the monofactor analysis was used to study the group differences of 16 biomarkers of multidimensional health variables in the middle-aged and elderly;thirdly,From the perspective of original information and the perspective of high-risk risk threshold setting of biomarkers,factor analysis was used to extract the potential factor physiological system of biomarkers;finally,the single physiological system of middle-aged and elderly organisms was estimated by logistic regression of categorical variables and OLS multiple linear regression.The correlation with multidimensional health status was verified to verify the relationship between AL levels in middle-aged and elderly people and multi-dimensional health status.In addition,multinomial logit regression was used to analyze the influence of middle-aged and elderly AL levels on multi-dimensional health changes in the extended study.Results:1.In the comparison of the health status of middle-aged and elderly people with different characteristics,it was found that there are significant differences in the body’s multiple biomarkers in the middle-aged and elderly self-reported health、mental health、activities of daily living、instrumental activities of daily living and cognitive function(P<0.100).In addition,there were differences in demographic characteristics such as age、gender and marriage among different health dimensions(P < 0.001).2.The study from the perspective of fully retaining the original information of biomarker data and the setting of two categories of biomarker high-risk threshold,the results are consistent through factor analysis,and six main factors are extracted namely “kidney metabolic system”factor、“blood lipid metabolic system”factor、“cardiovascular” factor 、 “blood glucose metabolic system”factor 、 “body measurement” factor and “inflammation and immune system” factor.3.After fully controlling the mixed factors such as demographic characteristics,socio-economic status,and middle-aged and elderly life behavior,the paper explores the relationship between the factor scores of the six main factors under the multisystem、the cumulative results of the abnormal biomarkers of the six main factors under the perspective of allostatic load,and the multidimensional health indicators of the middle-aged and elderly people,and the consensus conclusion was obtained,which is,factors such as "cardiovascular" physiological systems are related the self-reported health、 the mental health、 the activities of daily living、the instrumental activities of daily living and cognitive function of the middle-aged and elderly people(P<0.100).4.Allostatic load,as a comprehensive evaluation index of multifunctional disorder caused by long-term stress,in the correlation exploration with the multidimensional health status of middle-aged and elderly people,the study was found that AL and the self-reported health(< 0.001),the damaged of the activities of daily living(P value < 0.001),the damaged of the instrumental activities of daily living(P value < 0.001),and the reduction of cognitive function(P value < 0.05).In addition,the extended study of AL and multi-dimensional health changes in middleaged and elderly people further confirmed the significant impact of AL on the future multi-dimensional health changes in middle-aged and elderly people.Conclusion:The results of this study show that allostatic load is the relatively independent risk factor for multidimensional adverse health outcomes in middle-aged and elderly people,which confirms that the pathophysiological mechanism of physical and psychological multidimensional adverse health outcomes in middle-aged and elderly people.We are warned that we should strengthen regular physical examination for the middle-aged and elderly,promote the implementation of early intervention measures,maintain the health of the middle-aged and elderly in an allround and whole process,and promote the implementation of the policy of healthy aging.
