Distribution Characteristics Of Syndrome Elements In The Syndrome Drift Of Phlegm Turbidity Stagnation Syndrome In Nonalcoholic Fatty Liver Disease Complicated With Hyperuricemia

Objective:To observe the distribution and characteristics of syndrome drift in syndrome of phlegm-turbidity stagnation in NAFLD complicated with HUA.Grasp the changes of diseases from the micro constant dynamic level,and enrich the dialectical thinking of TCM.Methods:94 cases of syndrome of phlegm-turbidity stagnation in NAFLD complicated by HUA patients in Xiamen Hospital of Traditional Chinese Medicine from July 2020 to November 2021 were enrolled in this study.Follow-up would be ended when the syndrome drift occurs and up to three months.TCM diagnostic information was recorded every 2weeks.TCM diagnostic information were assessed by the Traditional Chinese Medicine Health Identification System software.The characteristics of syndrome element distribution were analyzed by SPSS 26.0 software package.Results:1.The syndrome drift occurred in 51(54.3%,95% CI:44.0%～64.5%)of the 94 patients included in this study.2.Baseline Data:The levels of BMI、ALT、AST、GGT、ALP、SUA、LDL-c、Apo B、CAP in the syndrome drift group were higher than those in the non-syndrome drift group(P<0.05).The levels of age in the syndrome drift group were lower than those in the non-syndrome drift group(P<0.05).3.After Logistic regression analysis,BMI,ALT,SUA,and CAP values are independe nt influencing factors of syndrome drift in syndrome of phlegm-turbidity stagnation in NAFLD complicated with HUA.4.The statistics analysis of syndrome elements which the score≥70 in the syndrome drift group:disease location syndrome elements were existed between pre-and post-syndrome drift include liver,spleen,kidney,gallbladder,stomach.The emerging disease location syndrome elements were sinews and bones,semi-exterior-interior,lung,meridian,heart spirit,heart,exterior,small intestine.Disease nature syndrome elements were existed between pre-and post-syndrome drift include dampness,phlegm,heat,qi stagnation,blood stasis,yin deficiency,dyspepsia,qi deficiency,yang deficiency.The emerging disease nature syndrome elements were cold,fluid retention,yang-hyperactivities,blood deficiency.5.The statistics analysis of all syndrome elements with points(whether or not≥70points)in the syndrome drift group:the disease location syndrome elements score of spleen,kidney,liver,semi-exterior-interior,stomach,gallbladder,meridian,semi-exterior-interior,lung,heart spirit was significant statistical difference between syndrome drift and non-syndrome drift group(P<0.05).The disease location syndrome elements were existed between pre-and post-syndrome drift include spleen,kidney,liver,stomach,gallbladde,semi-exterior-interior,heart spirit.The emerging disease location syndrome elements were sinews and bones,meridian,lung.The disease nature syndrome elements score of dampness,phlegm,heat,blood stasis,qi stagnation,yin deficiency,cold,qi deficiency,fluid retention,yang-hyperactivities,blood deficiency,qi Collapse was significant statistical difference between syndrome drift and non-syndrome drift group(P<0.05).The disease nature syndrome elements were existed between pre-and post-syndrome drift include dampness,phlegm,heat,blood stasis,qi stagnation,yin deficiency,qi deficiency,fluid retention,yang-hyperactivities,qi collapse.The emerging disease nature syndrome elements were blood deficiency,cold.Conclusion:1.Syndrome drift was common in syndrome of phlegm-turbidity stagnation in NAFLD complicated by HUA,and 54.3%(95% CI:44.0%～64.5%)of the patients developed syndrome drift during the 3-month follow-up period.Patients with high levels of BMI,ALT,AST,GGT,ALP,SUA,LDL-c,Apo B and CAP values and younger patients were more prone to syndrome drift.BMI,ALT,SUA and CAP values were independent influencing factors of syndrome drift in syndrome of phlegm-turbidity stagnation in NAFLD complicated with HUA.Clinically,patients should be guided to eat a reasonable diet and exercise,and closely monitor the changes of the above research indicators.2.According to the statistics of syndrome elements in the syndrome drift group with a score of ≥70 points and no matter if they have a score of ≥70 points.the disease location syndrome elements liver,spleen,kidney,gallbladder,and stomach,and the disease nature syndrome elements dampness,phlegm,heat,blood stasis,qi stagnation,qi deficiency,yin deficiency,yang deficiency were throughout.The emerging disease location syndrome elements were sinews and bones,meridian,lung and the emerging disease nature syndrome elements were blood deficiency,cold.Actively seek effective compound prescriptions in treatment,adjust the compatibility of traditional Chinese medicine as soon as possible,and deal with the subtle changes of the disease.
