Clinical Observation Of Shentong Zhuyu Decoction Combined With Endoscopic Spinal Canal Expansion In The Treatment Of Lumbar Spinal Stenosis

Objective:To observe the clinical effect of Shentong Zhuyu Decoction combined with spinal endoscopic laminoplasty and decompression in the treatment of lumbar spinal stenosis(syndrome of qi stagnation and blood stasis).Methods:The same diagnosis and treatment group of the Spine Surgery Department of the Second Hospital of Fuzhou City(the main surgeon was the same surgeon)were selected,and during the period from January 2021 to December 2021,endoscopic spinal canal expansion and decompression was performed due to lumbar spinal stenosis.60 surgical patients were randomly divided into two groups,the control group was treated with endoscopic spinal canal enlargement and decompression surgery,and the experimental group started to take body weight on the basis of the control group from the second day after operation.Shentong Zhuyu Decoction was used for 3 weeks.The VAS score,JOA score and ODI score,the dosage of celecoxib and the TCM syndrome score before surgery,1 week,3 weeks,and 6 weeks after surgery were recorded and analyzed in the two groups of patients.SPSS26.0 software for analysis and statistics.Results:1.A total of 60 patients were included in this trial.Among them,1 patient in the experimental group underwent open surgery for fracture of the spondylotic isthmus after physical exertion 1 month after surgery.59 patients were finally completed,and all patients had no adverse effects.React events.There was no significant difference in the general data(sex,age,course of disease,treatment segment,operation time,intraoperative blood loss)between the two groups before treatment(P>0.05),and they were comparable.2.There was no significant difference in the preoperative VAS score,JOA score,ODI score,TCM syndrome score and other observation indicators between the two groups(P>0.05),and the two groups were comparable.3.The intra-group comparison of the two groups showed that the postoperative VAS scores of the patients were lower than those before the operation,and the difference was statistically significant(P<0.05).There was no significant difference in VAS scores between the two groups at all time points after surgery(P>0.05).4.In the comparison between the two groups,the postoperative JOA scores of the patients were all higher than those before the operation,and the difference was statistically significant(P<0.05).The JOA score of the group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05).5.The intra-group comparison of the two groups showed that the postoperative ODI scores of the patients were all higher than those before the operation,and the difference was statistically significant(P<0.05).The ODI score of the group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05).6.Comparing between the two groups,the dosage of Celebrex in the experimental group was significantly less than that in the control group on the 2nd day after operation and at the following time points,and the difference was statistically significant(P<0.05).7.Comparison between the two groups,6 weeks after the operation,the TCM syndrome score of the experimental group was significantly lower than that of the control group,and the difference was statistically significant(P<0.05).The symptoms of all patients were improved,the effective rate of the experimental group was 93.1%,and the effective rate of the control group was 83.3%,and the difference was statistically significant(P<0.05),indicating that the overall curative effect of the experimental group was better than that of the control group.Conclusion:Shentong Zhuyu Decoction combined with endoscopic laminoplasty and decompression therapy and simple laminoplasty and decompression for lumbar spinal stenosis can improve the symptoms of patients to different degrees.Among them,Shentong Zhuyu Decoction Combined surgical treatment is more effective in restoring lumbar spine function and daily social life ability,and improving the quality of life of patients,and Shentong Zhuyu Decoction combined with surgical treatment can improve the pain in the lower back and legs of patients and reduce the demand for postoperative analgesics.This scheme helps to improve the efficacy,is safe and practical,and can provide a reference for the selection of clinical treatment schemes.
