| Objective: To evaluate intratumoral inflammation and epithelial mesenchymal transformation(EMT)of vestibular schwannomas(VSs)by postoperative pathological examination,and explore the correlation between intratumoral inflammation and EMT.Methods: A total of 40 PATIENTS with KOOS grade 3-4 VSs who received microsurgery in the Neurosurgery Department of Sichuan Provincial People’s Hospital from October 20,2020 to October 20,2020 were selected.During the operation,the free part of tumor(posterior part of tumor in cerebellar pontine cistern),the central part of tumor,and the contact part of tumor nerve structure(including facial nerve and brainstem)were selected from each patient.Immunohistochemical methods were used to evaluate the overall inflammatory degree of VSs tumor tissue by CD45 detection,IL-6 and TGF-βexpression to evaluate inflammatory factors,E-cadherin,N-cadherin,Vimentin,β-catenin to evaluate EMT,SPSS 26.0 was used for statistical analysis of the data to analyze the correlation between inflammation indicators and EMT indicators,and P < 0.05 was considered statistically significant.Results :(1)the positive expressions of CD45,IL-6 and TGF-β were found in 40 cases(100%)by VSs immunohistochemical staining.CD45,IL-6 and TGF-β were higher in the contact part of nerve structure than in the free part and the central part of tumor,and the difference was statistically significant(P < 0.05).(2)There were 26 cases of cystic solid VSs and 14 cases of solid VSs.The inflammatory indexes of CD45,IL-6 and TGF-β in the cystic solid group were higher than those in the tumor free part,central part and nerve structure contact part,but the differences were not statistically significant(P > 0.05).(3)Koos grade 3(maximum diameter 2-3cm)VSs22 cases,Koos grade 4(maximum diameter > 3cm)VSs 18 cases,Koos4 group inflammation index CD45,IL-6,TGF-β in the tumor free part,central part,nerve structure contact part of the three groups were heavier than Koos3 group.But it did not reach statistical significance(P > 0.05).(4)The expression levels of N-cadherin,β-catenin and Vimentin in the contact part of the nerve structure of 40 VSs tumors were significantly higher than those in the free part and central part of the tumor(P< 0.05),E-cadherin in the contact part of nerve structure was significantly lower than that in the free part and central part of tumor(P<0.05),and there was no statistical significance between the free and central parts(P > 0.05).(5)Spearman correlation analysis showed that there was a significant positive correlation between CD45 and N-cadherin and Vimentin in 40 cases of tumor nerve structure contact(correlation coefficient r > 0,P < 0.05),there was no significant correlation between CD45 and E-cadherin and β-catenin(P>0.05);There was a significant positive correlation between IL-6 and Vimentin(correlation coefficient r > 0,P< 0.05),there was no significant correlation between IL-6 and e-cadherin,β-catenin and N-cadherin(P > 0.05);TGF-β was positively correlated with N-cadherin and vimentin(correlation coefficient r > 0,significance P < 0.05),there was no significant correlation between TGF-β and e-cadherin and β-catenin(significant P>0.05).Conclusion :(1)inflammation is one of the characteristics of VSs microenvironment,and the degree of inflammation in the contact part of tumor nerve structure is more serious than that in the free part and central part of tumor.(2)EMT exists in the nerve structure contact of VSs tumor.(3)VSs inflammation had a synergistic effect on EMT,in which IL-6 and TGF-β were involoved... |
